Abstract |
Durant de nombreuses années, les enfants de familles touchées par l’alcoolodépendance ont été à peine pris en considération dans la recherche scientifique, et, en raison du manque de discours sur leur situation, le grand public était également peu conscient du problème. En 1969 pourtant, Cork rapportait dans “The forgotten children” divers symptômes frappants qui étaient apparus chez ces enfants. A la fin des années 80, des auteurs tels que Black (1988), Wegscheider (1988) et Lambrou (1990) ont formulé pour la première fois des modèles théoriques pour l’alcoolisme en tant que maladie familiale. Ils décrivent comment l’alcoolisme de l’un ou des deux parents confère à l’alcool une place centrale dans la famille, de quelle manière cela influence l’ambiance à la maison, et quels sont les effets de cette situation sur les enfants et leur vie quotidienne. Alarmés par ces premières publications scientifiques, les spécialistes ont pris conscience de la nécessité de pousser plus loin la recherche et de fournir une aide pratique aux enfants. C’est dans ce contexte que s’est formée en 1983, aux Etats-Unis, la “National Association for children of alcoholics” (NACoA). Cette organisation a pour but de s’investir dans les intérêts d’enfants dont la famille est touchée par l’alcoolisme ou d’autres formes d’addiction. La NACoA (entre-temps aussi implantée en Grande-Bretagne et en Allemagne) s’est donné pour mission d’attirer l’attention du grand public sur ces enfants grâce à une meilleure information et d’évoquer la dépendance en tant que maladie familiale. En Europe, le « European Network for Children Affected by Risky Environments within the Family » (ENCARE) a été fondé en 2002 dans le but de lancer le débat sur la problématique des enfants de familles touchées par une addiction, de stimuler la recherche sur ce thème et de former un réseau de spécialistes. Actuellement, des institutions de 23 pays européens sont représentées dans ENCARE et il existe des réseaux régionaux au niveau national. Grâce à la sensibilisation du monde professionnel et dans le cadre des réseaux nouvellement créés, le nombre de spécialistes qui s’occupent de ces enfants a augmenté, en Suisse comme dans les pays voisins. Depuis quelques années, on dispose également d’expériences pratiques liées aux interventions et aux traitements, bien qu’elles ne soient que ponctuelles et disponibles dans un nombre limité de cantons. |

Abstract |
New drugs were detected in the European Union last year at the rate of around one per week, according to the EMCDDA–Europol 2011 annual report on new psychoactive substances released today. A total of 49 new psychoactive substances were officially notified for the first time in 2011 via the EU early-warning system (EWS). This represents the largest number of substances ever reported in a single year, up from 41 substances reported in 2010 and 24 in 2009. |

Abstract |
The prescription of substitution drugs, such as methadone and buprenorphine, has become a mainstream, first-line treatment for opioid dependence, with around 700 000 of Europe’s 1.3 million problem opioid users receiving substitution treatment today. But a small minority of entrenched opioid users repeatedly fails to respond to interventions of this kind. Findings from international trials now suggest that the supervised use of medicinal heroin can be an effective second-line treatment for this small, and previously unresponsive, group. In this latest EMCDDA Insights report, experts describe the development as “an important clinical step forward”. The report provides the first state-of-the-art overview of research on the subject, examining the latest evidence and clinical experience in this area in Europe and beyond. |

This study brings together available evidence to provide a comprehensive analysis of cannabis production and markets across the EU. It combines information from EMCDDA routine reporting – data on patterns of prevalence and use, seizures, police reports, drug-law offences, cannabis potency and retail market prices – with literature on cannabis markets to create an in-depth analysis of the issue in a European context. |

Abstract |
In order to help drug users become full members of society following treatment, measures are needed that address the issues of housing, education, vocational training and employment as part of their recovery. This report considers existing interventions targeting this vulnerable social group. It also provides a set of conclusions targeted at policymakers and drug practitioners, in order to help them develop coherent and comprehensive social integration strategies. Examples of ‘what works’ in practice are a vital first step in developing evidence-based guidelines for future interventions. |

Abstract |
This Manual is a revised edition of the Treatment demand indicator (TDI) protocol version 2.0 and presents updated guidelines for reporting data on people entering drug treatment in Europe. The TDI is one of five key epidemiological indicators which provide a common European methodology for collecting and reporting core data on the number and profiles of those entering specialised drug treatment each year. TDI data are routinely used in EMCDDA analysis of the drug situation in Europe, helping to identify trends and patterns in problem drug use and to assess the use and uptake of treatment facilities. |

Abstract |
The European School Survey Project on alcohol and Other Drugs (ESPAD) is a collaborative effort of independent research teams in more than forty European countries, making it the largest cross-national research project on adolescent substance use in the world. ESPAD was founded in 1993, on the initiative of the Swedish Council for Information on alcohol and Other Drugs (CAN) and with the support of the Pompidou Group at the Council of Europe. In later years, ESPAD has also established co-operation with the European Monitoring Centre for Drugs and Drug Addiction (EMCDDA), a body of the European Union. Most of the European continent is now covered by ESPAD, meaning that it provides a reliable overview of trends in substance use among 15-16-year-old European students. Data are collected every four years. This report presents the results from the fifth wave, conducted in 36 countries during spring 2011. It gives a comprehensive picture of the present situation among European young people as regards the use of tobacco, alcohol, cannabis and other substances, as well as an overview of trends in 1995-2011. |

Abstract |
Die vorliegenden “Medizinischen Empfehlungen für substitutionsgestützte Behandlungen (SGB) bei Opioidabhängigkeit” wurden durch die Schweizerische Gesellschaft für Suchtmedizin, SSAM, im Auftrag des Bundesamtes für Gesundheit, BAG, erarbeitet. Sie ersetzen die Empfehlungen vom 19.06.2007. Es liegen eine ausführliche Version mit Referenzierung und eine Kurzversion vor. Nach der Veröffentlichung der Empfehlungen im Jahr 2007 wurde im Jahr 2009 ein weiterer Meilenstein erreicht, der medizinisch und politisch wesentlich zur Koordination und Qualitätsverbesserung der SGB in der Schweiz beitrug. Eine Kurzversion der SSAM Empfehlungen wurde gemeinsam mit dem BAG und der Vereinigung der Kantonsärzte der Schweiz (VKS) herausgegeben. Sie diente in der Folge als offizielle Grundlage für substituierende Ärzte, für die Übernahme der Behandlungskosten durch die Krankenkassen und als Basis für standardisierte Empfehlungen für kantonale Behandlungsrichtlinien. Für Grundversorger wurde ein Handbuch auf der Plattform www.praxis-suchtmedizin.ch geschaffen. Seither hat sich der Stellenwert der SGB als Behandlung erster Wahl bei Opioidabhängigkeit mit fundierter Evidenz in internationalen Guidelines weiter gefestigt (Weltgesundheitsorganisation WHO, World Federation of Societies of Biological Psychiatry WFSBP, National Institute for Health and Clinical Excellence NICE). Die Opioidabhängigkeit wird heute als chronische Erkrankung und damit als Langzeitgeschehen verstanden. Nur eine Minderheit der Abhängigen erreicht eine dauerhafte Abstinenz. Verlässliche Prädiktoren für das Erreichen einer anhaltenden Abstinenz oder die Wirksamkeit therapeutischer Massnahmen zur Verbesserung der Abstinenzrate sind nicht beschrieben [1,2,3]. Analog zu anderen chronischen Erkrankungen, wird die SGB nicht mehr als Sonderfall einer Behandlung, sondern als «normale» medizinische Therapie mit Berücksichtigung aller Begleitumstände betrachtet. Die Zahl der Opioidabhängigen in der Schweiz wird vom BAG auf ca. 22-27’000 geschätzt. Ungefähr 19’400 Opioidabhängige befanden sich im Laufe des Jahres 2010 in einer SGB, ca. 1400 substituiert mit Diazetylmorphin, von den weitern 18’000 waren über 90% mit Methadon substituiert, mit Buprenorphin im mittleren einstelligen Prozentbereich und ca. 1% mit anderen Opioiden [4]. Die Forschung zeigt, dass die Anzahl neuer Opioidabhängiger (Inzidenz) in der Schweiz stark rückläufig und die «Heroinepidemie» der späten 1980er und frühen 1990er Jahre somit am Abklingen ist [4,5,6]. Trotz sinkender Inzidenz und geringerer Prävalenz (die Abgänge wegen Todesfällen und Ausstieg in die Abstinenz werden durch die Neuzugänge nicht ersetzt) blieb die Anzahl der Substituierten in den vergangenen Jahren stabil. Der prozentuale Anteil der Behandelten stieg also deutlich an. Wo immer möglich, stützen sich die Empfehlungen auf die erwähnten, anerkannten aktuellen internationalen Guidelines zur SGB [1,2,3], auf Metaanalysen und insbesondere auf Cochrane Übersichtsarbeiten (www.thecochranelibrary. com). Kapitelweise ist eine Selektion der aktuellen und relevanten Publikationen ab 2007 aufgeführt. Frühere Arbeiten sind nur ausnahmsweise zitiert und referenziert, die weiteren relevanten älteren Arbeiten finden sich in den Referenzlisten der oben angeführten Publikationen. Abstract |
Das kantonale Alkoholpräventionsprogramm definiert die kantonale Strategie in der Alkoholprävention für die nächsten Jahre. Ein Schwerpunkt des Programms stellt die Auseinandersetzung der Bevölkerung mit dem eigenen Konsumverhalten dar. Weitere Schwerpunkte des Programms sind die Verminderung des chronischen Risikokonsums im Alter, die Unterstützung von Kindern aus suchtbelasteten Familien sowie ein besser greifender Jugendschutz. Das bestehende Angebot wird mit weiteren Massnahmen ergänzt werden. So können beispielsweise Spitexmitarbeitende und andere Personen, welche mit älteren Menschen in Kontakt stehen, eine Schulung zum richtigen Vorgehen bei Verdacht auf Alkoholprobleme besuchen. Oder es werden Gesprächsgruppen für Jugendliche aus suchtbelasteten Familien geschaffen, damit diese sich austauschen können. Das Programm vernetzt sämtliche Institutionen, welche in der Alkoholprävention im Kanton Solothurn tätig sind und koordiniert die Angebote. Damit wird ein zielgerichtetes, gemeinsames Vorgehen gegen den risikoreichen Alkoholkonsum gewährleistet. |
