Zahlreiche psychoaktive Medikamente weisen ein Missbrauchspotential auf. Aufgrund ihrer primär heilenden Funktion sind die Mechanismen der Missbrauchsentwicklung zum Teil anders als bei Genussmitteln oder illegalen Drogen. Die letzten Zahlen der Schweizerischen Gesundheitsbefragung SGB von 2007 deuten auf eine hohe Verbreitung der täglichen Einnahme von Schlaf-, Beruhigungs- und Schmerzmitteln bei Frauen und älteren Personen hin. Massnahmen in Bezug auf die Verschreibungsregelungen sollten erwogen werden |

Zahlen der Schweizerischen Gesundheitsbefragung 20121 zeigen, dass 8.9% der Schweizer Bevölkerung ab 15 Jahren mit einem chronischen Alkoholkonsum, von im Mittel mehr als 40 Gramm reinen Alkohols pro Tag für Frauen und mehr als 60 Gramm für Männer, einen zu hohen Blutdruck berichteten. Hingegen war dies nur bei 4.0% der Lebenszeitabstinenten der Fall. Auch unter Kontrolle weiterer Risikofaktoren blieb der Zusammenhang zwischen Alkoholkonsum und Bluthochdruck bestehen. |

Wurde störende Verschiedenheit lange Zeit mit der Begründung der Krankhaftigkeit oder Abnormalität systematisch bekämpft, wird seit Mitte des letzten Jahrhunderts in zunehmendem Masse die Medikalisierung sozialer Problemlagen als Mittel der Disziplinierung eingesetzt. Als neue Disziplinierungsnorm gilt nicht mehr die Orientierung an der gesellschaftlichen Homogenität in einem bestimmten Territorium des Einflusses, sondern jene des vernünftigen Pluralismus, der sich territorial kaum mehr fassen lässt. |

Wo dürfen Ausländerinnen und Ausländer in der Schweiz an die Urne? Wo gibt es eine Stadtpräsidentin, die keinen Schweizer Pass vorzuweisen hat? Welche Kantonspolizei beschäftigt Ausländerinnen? Welches Richteramt verlangt nicht das Schweizer Bürgerrecht? Welcher Kanton verfügt über ein Integrationsgesetz, welcher über ein Leitbild zur Integration? Wie können Ausländer-Organisationen bei Vernehmlassungen mitwirken? Das föderale System der Schweiz macht es ziemlich schwierig, Fragen zu den Mitwirkungs- und Mitgestaltungsmöglichkeiten von Ausländerinnen und Ausländern zu beantworten. Die Zusammenstellung “Mitreden und Mitgestalten – Strukturelle Partizipation in den Kantonen” vergleicht die Partizipationsformen in den verschiedenen Kantonen. Sie regt an, Modelle aus anderen Kantonen zu prüfen und im eigenen Kanton neue Mitwirkungsmöglichkeiten ins Auge zu fassen. Où les étrangers peuvent-ils se rendre aux urnes dans notre pays? Où y a-t-il une présidente de municipalité qui n’a pas de passeport suisse? Quelle police cantonale emploie des étrangers? Quelle magistrature cantonale n’exige pas que le juge ait la nationalité suisse pour exercer ses fonctions? Quel canton dispose d’une loi sur l’intégration et lequel dispose d’une charte sur l’intégration? Comment les organisations des étrangers peuvent-elles participer à la procédure de consultation? En raison du système fédératif de notre pays, il est difficile de répondre à ces questions ou à des questions analogues concernant les possibilités de coopération et de codécision des étrangers. L’étude “Se responsabiliser et s’engager: Participation structurelle dans les cantons” compare les formes de participation dans les différents cantons. Elle vise à examiner les modèles dans d’autres cantons et à envisager de nouvelles possibilités de participation dans son propre canton. |

Within the Swiss Detoxification Coordination (SwiDeCo) framework, a number of randomised clinical trials on opiate detoxification took place, comparing different settings (in- and outpatient), medications (antagonists under anaesthesia, methadone, buprenorphine high/low dosage) and duration of treatment (fixed and flexible). A standard assessment with validated instruments and standard follow-up was used throughout all the studies. One study concerned neonatal opiate detoxification. The scientific framework offered by SwiDeCo provided an opportunity for links and exchange between different drug treatment centres (over 20 centres participated in the studies) and persons working in the field of substance abuse in Switzerland. Overall, 279 patients were included in the different trials, of whom three-quarters were male, on average 30 years old; multi substance abuse was frequent. All procedures to withdraw patients from opiates tested in the different trials resulted in an equally poor outcome regarding abstinence after 3 months (22%). There were no patient baseline characteristics predicting differences in outcome. However, in-patient detoxification resulted in higher short-term abstinence rates, but this advantage disappeared over time. Patients’ level of distress at baseline evaluation decreased independently of treatment outcome during the first three months. Therefore, there seems to be a beneficial, but outcome-independent, treatment initiation effect. Two patients died within 3 months of detoxification. Although we cannot formally conclude whether withdrawal treatment resulted in an excess mortality, this fact is disquieting. During the detoxification phase overdose prevention should be discussed. Finally, based on our study results and on similar results from other groups, we cannot recommend the UROD procedure for withdrawal. We would rather suggest engaging patients who insist on this treatment modality in outpatient treatment, where tapering of substitution opiates could be performed while monitoring resumption of heroin consumption or the consumption of other substances. Given the high relapse rates independent of the specific detoxification treatment offered, it is important to offer flexible treatment goals to patients undergoing withdrawal treatment, so that substitution treatment might be offered in case of failure. The technique is simple and can be applied by trained family doctors. |

With the limited exception of HIV/AIDS prevention, harm minimisation in the UK has focused on the harm that drug users may do to themselves. This article explains how harm minimisation within Hammersmith and Fulham (part of inner London ) has now begun to address wider issues. Hammersmith and Fulham has the highest estimate HIV+ rate of any local authority area in the UK, and approximately 4000 injecting drug users. Spending on HIV prevention and services for people with HIV/AIDS is the highest in the UK . Police estimate that more than half of all acquisitive crime in the area is committed to fund drug purchases. Public services in the health, criminal justice, education, housing and social fields have formed an unusually close partnership to address these problems. This is generating rapid changes in service provision, including moves away from prosecution of people in possession of any drug for their own use, and moves to attract many more users into treatment by greatly extending prescribing practice. The overall aims of harm minimisation in the area now include reduction of crime and reduction of the illicit drug market alongside minimisation of harm to drug users themselves. |

With over 10 million visitors per year, Erowid.org’s reputation for providing reliable, non-judgmental information places it at a crossroads between the users of novel psychoactive drugs and public health efforts. In the last decade, the Erowid Experience Vaults have become a popular source of data about an increasing range of psychoactives. Advantages and disadvantages of these self-reports are discussed, with a focus on the challenges posed by the use of new stimulants, empathogens, cannabinoids, and psychedelics that are hard to control, insufficiently researched, and often of uncertain identity. |

Wissenschaftliche Studien belegen, dass risikoreiches Trinken und damit verbundene Probleme umso geringer sind, je eingeschränkter die Erhältlichkeit von Alkohol ist. Zur Verdeutlichung der Rahmenbedingungen und vermittelnder Faktoren wird ein theoriegeleitetes Modell vorgestellt. Aus der Sicht evidenzbasierter Prävention sollte die Besteuerung angehoben, die Einhaltung von Alters- und Ortsbeschränkungen überprüft und Öffnungszeiten eingeschränkt werden. Erwachsene und ältere Jugendliche sollten motiviert werden, keinen Alkohol an unter 16-Jährige abzugeben. Darüber hinaus sollten Eltern restriktive Regeln aufstellen. |
