Abstract (down) Research is an increasing priority for workers throughout the mental health sector, and therapeutic communities are no exception. Those working in TCs increasingly have to justify the success and efficiency of their methods to outside bodies, and the prime means of doing so is through research. This volume collects a wide range of papers by experienced contributors discussing all aspects of TC research. They consider questions of which methods are most appropriate in the unique environment of TCs, how research studies affect the TC environment, as well as practical and ethical questions. The book also includes accounts of several research studies undertaken at, among other places, the Cassel Hospital. Taken as a whole, the book will be an invaluable resource for anyone involved in researching therapeutic communities – whether undertaking a study, attempting to anticipate its clinical impact, or seeking to understand the kinds of results which TC research produces.

Abstract (down) Regelmäßiges Alkoholtrinken ist bei Jugendlichen seltener geworden als noch vor fünf Jahren. Dafür ist der exzessive Konsum in einer abgrenzbaren Risikogruppe nach wie vor Besorgnis erregend. Für Jugendliche gibt es „gute Gründe“ zum Alkoholkonsum und entwicklungsbedingt wenig Problembewusstsein für die Risiken. Es ist von daher Aufgabe der Erwachsenen, Konsummöglichkeiten zu begrenzen und für ein spätes Einstiegsalter zu sorgen. Präventionsziel ist der risikoarme Umgang mit Alkohol.

Sie enthält die wichtigsten eidgenössischen und kantonalzürcherischen Gesetzestexte zum Verkauf und zur Gratisabgabe von Alkohol und Tabak sowie zum Schutz vor Passivrauchen. Zudem enthält sie gesetzliche Bestimmungen und behördliche Regelungen zu Alkohol und Tabak im schulischen Bereich. Die Sammlung ist nicht vollständig. Insbesondere Regelungen bezüglich Werbung für Alkohol- und Tabakprodukte sind nur sehr lückenhaft aufgeführt. Wenn ein Sachverhalt sowohl eidgenössisch als auch kantonal geregelt wird, gilt jeweils die strengere der beiden Regelungen.

Abstract (down) Recent years have witnessed a proliferation of new psychoactive substances becoming available in Europe. This can be illustrated through the rise in notifications of new substances to the EU early warning system, from just 14 per year in 2005 to 73 in 2012. Some of these substances will find their way on to the market, packaged and promoted as ’natural’ or ’legal’ products, in specialised physical and online shops. In particular, Internet sales of new drugs means that the expanding global network cuts across national boundaries and jurisdictions.

Abstract (down) Recent major epidemiological surveys in general population samples throughout the world have demonstrated that substance use disorders are among the most frequent forms of mental disorders in the community, and are also frequently associated with other forms of mental disorders. This paper briefly reviews the concept of comorbidity and summarizes more recent data concerning the frequency of comorbidity of substance use disorders. The review is limited to studies in the general population using standardized diagnostic interviews. Specific emphasis is laid upon the most recent data from the National Comorbidity Survey, that specifically addressed comorbidity issues in detail. The clear majority of subjects with a definite substance use disorder according to the strict DSM-III-R definition has or has had at least one other comorbid mental disorder. Comorbidity rates between specific substance use disorders and other mental disorders will be compared and discussed in light of several other international epidemiological studies. Furthermore time sequences of substance use disorders and comorbid disorders are presented. Potential pathogenetic and clinical implications are addressed.

Abstract (down) Recent estimates suggest that, each year, over 1 million Europeans receive treatment for problems related to the use of illicit drugs (EMCDDA, 2010). Of these, more than half receive opioid substitution treatment for problems related to opioid drugs, primarily heroin. The current levels of provision of drug treatment services in the European Union are the result of an expansion of services that has taken place during the last two decades. For much of that time, services expanded against a background of a growing drug problem, and one in which economic conditions were largely favourable. Now, however, in an era of cuts in government expenditure, managing the costs of treatment and ensuring the highest quality and best outcomes for the lowest possible cost are priorities. While measuring and improving drug treatment outcomes have been relatively high on the research agenda in recent years, the cost of treatment has received far less research attention. Addressing this information gap is likely to have a number of benefi ts for both decision-makers and treatment providers. On the one hand, service providers need accurate information on the costs of service provision in order to plan the allocation of resources. On the other, decision-makers and funders can use such information as a means of cost control, for example, by comparing costs of similar services or those of alternative providers of similar services. Finally, as a part of a fuller economic analysis, information on service costs is needed to determine the cost-effectiveness of interventions and treatment programmes. The cost of drug treatment in Europe is looked at from two main angles in this Selected issue. First, it presents an overview of the main sources of drug treatment funding and the financing mechanisms that are employed in European countries. The available data are presented on who is paying for drug treatment, how funding is organised, and how funding is distributed among different treatment modalities. This is followed by an analysis of the unit costs of different types of drug treatment. Costs related to opioid substitution treatment including the costs of substitution medication for a number of European countries are presented here. This publication keeps a tight focus on the cost of drug treatment and does not attempt to undertake a broader economic analysis of the healthcare and social costs relating to illicit drugs. This Selected issue represents a fi rst attempt to provide a European overview of drug treatment costs, and individual countries are brought into focus to illustrate specifi c aspects of the analysis. This subject is both topical and methodologically challenging. The varying availability of information as well as the complexity of funding arrangements mean that, at best, only very incomplete estimates can be made of the costs of (or expenditure on) drug treatment in Europe.

Abstract (down) Recent decades have seen a growth in travel and tourism abroad because of cheap air fares and holiday packages. This has been accompanied by a relaxation of border controls, especially within parts of Europe participating in the Schengen Agreement. As some people may be more inclined to use illicit substances during holiday periods and some may even choose to travel to destinations that are associated with drug use – a phenomenon sometimes referred to as “drug tourism” – this means that from a European drug policy perspective the issue of drug use and travel has become more important.

Abstract (down) Recent advances in prevention science provide evidence that adolescent health and behavior problems can be prevented by high-quality prevention services. However, many communities continue to use prevention strategies that have not been shown to be effective. Studying processes for promoting the dissemination and high-quality implementation of prevention strategies found to be effective in controlled research trials has become an important focus for prevention science. The Communities That Care prevention operating system provides manuals, tools, training, and technical assistance to activate communities to use advances in prevention science to plan and implement community prevention services to reduce adolescent substance use, delinquency, and related health and behavior problems. This paper describes the rationale, aims, intervention, and design of the Community Youth Development Study, a randomized controlled community trial of the Communities That Care system, and investigates the baseline comparability of the 12 intervention and 12 control communities in the study. Results indicate baseline similarity of the intervention and control communities in levels of adolescent drug use and antisocial behavior prior to the Communities That Care intervention. Strengths and limitations of the study’s design are discussed.
