Quels sont les effets et les risques sur le plan physique et psychique auxquels on peut s’attendre en cas de consommation épisodique ou régulière? En matière de prévention et de réduction des risques, de quels éléments faut-il tenir compte?

Abstract: Phase I of the Zurich Prometheus Study is a cross-sectional study focusing on an up-to-date serology for HIV and hepatitis B/C and associated risk factors for all clients in four participating clinics offering opiate substitution in Zurich, Switzerland. The mean age of the 603 respondents is 30.7 years (SD=6.2), and 38% of them are women. Seventy-five percent of the respondents have a history of injecting drug use (IDU), and over half have injected within the past six months. Lab-confirmed seroprevalence for HBV (50%) and HCV (57%) is twice that of HIV (24%). There is an 80% risk reduction for all three viral infections among those starting IDU after 1991—when harm reduction efforts were in full swing—compared to those who began before 1988—before clean needles were widely available. These findings suggest a strongly protective effective of harm reduction measures. But while a stabilization in HIV prevalence at 15% can be seen among drug users who started injecting after 1991, prevalence rates for HBV and HCV still remain several times higher. The prevalence data in this study support data showing continued high incidence rates for HBV and HCV, even among new injectors in the harm reduction era.

Abstract: Pharmakologisches Neuro-Enhancement steht für die nicht medizinisch indizierte Verabreichung und Anwendung von Medikamenten, die ursprünglich für Krankheitsbilder wie Aufmerksamkeits-Hyperaktivitätssyndrom (ADHS), Narkolepsie, Depression oder De-menz entwickelt wurden. Es geht darum, dass gesunde Menschen Stimulanzien, Antidementiva oder Antidepressiva zur Steigerung und Verbesserung ihrer kognitiven und emotionalen Funktionen einnehmen. Ursprünglich vor allem im Studentenmilieu verbreitet, scheint sich der Anwenderkreis immer mehr auf die «Normalbevölkerung» auszu-dehnen. Dahinter stehen zum einen arbeitsbezogene Motive (Arbeit bewältigen können, die Arbeit effizienter erledigen können, länger konzentriert arbeiten können etc.), zum anderen dominiert der Wunsch nach Selbstoptimierung und der Regulierung uner-wünschter Emotionen. Die Befunde zur Wirksamkeit von Psycho- und Neuropharmaka bei Gesunden sind bis-lang eher ernüchternd bzw. widersprüchlich. Dennoch ist ein beachtlicher Teil der Bevöl-kerung dazu bereit, pharmakologisches Neuro-Enhancement zu betreiben, wenn die Substanzen sicher und frei zugänglich wären. Die Errungenschaften medizinischer Hirn-Forschung, die Anforderungen der Leistungsgesellschaft, die demografische Entwick-lung (Menschen werden immer älter), die zunehmende Pharmakologisierung des Alltags sowie der Medikamentenbezug via Internet sind dafür wesentliche Wegbereiter. Man ermittelte bisher einen Anteil von 4-7% der Studierenden (USA) bzw. knapp 1% (Deutschland) sowie 5% der Angestellten zwischen 20 und 50 Jahren, die mindestens einmal Medikamente zu Enhancement-Zwecken eingenommen haben. Gefährlich sind die allfälligen Nebenwirkungen, die möglichen Langzeitschäden, das Suchtpotenzial sowie die Risiken des unkontrollierten Medikamentenbezugs via Internet. Die gesellschaftliche Diskussion um das Thema Neuro-Enhancement steht noch am Anfang. Hoffnungsvoller Fortschrittsglaube trifft dabei auf abwehrende Skepsis. Das Thema berührt zentrale ethische Fragen wie beispielsweise nach der Sicherheit, der sozialen Gerechtigkeit, des fairen Wettbewerbs, des gesellschaftlichen Drucks oder der Auswirkungen auf die Persönlichkeit. Welche Regelungen in Zukunft erforderlich sein werden, darum wird noch gerungen. Die Suchtprävention wird die Entwicklung in Bezug auf das pharmakologische Neuro-Enhancement weiter verfolgen und über ihre Rolle und Aufgabe in diesem Themenfeld diskutieren müssen. Sie muss sich insbesondere verstärkt mit dem Phänomen der verbreiteten Selbstmedikation im Alltag sowie dem Medikamentenmissbrauch auseinander-setzen. Dabei gerät besonders eine Substanz in den Fokus: Ritalin, das sowohl zur Steigerung der Konzentration und Wachheit als auch als aufputschende Party-Droge eingenommen wird.

Abstract: Personne n’est né joueur de jeu de hasard et d’argent. Comment en vient-on à participer à ce type d’activité ? Trois familles interreliées de déterminants jouent dans l’apparition du comportement. Premièrement, une personne doit avoir les capacités physiques et mentales pour s’adonner à ce type d’activité. Deuxièmement, des disponibilités sont requises. De fait, il faut que les jeux de hasard et d’argent soient accessibles, il faut qu’une personne dispose de quoi miser (le plus souvent de l’argent) et du temps requis pour participer. Troisièmement, les représentations qu’une personne entretient à propos de ces jeux doivent être suffisamment positives : les jeux doivent apparaître légitimes et le joueur doit en concevoir et en espérer une utilité. La présence simultanée des trois déterminants est nécessaire pour qu’une personne joue; elle n’est cependant pas suffisante à l’apparition du comportement. Pourquoi continue-t-on de jouer ? Les mêmes déterminants fonctionnent essentiellement de la même manière. À ceux-ci nous devons ajouter les caractéristiques intrinsèques des jeux qui favorisent la réitération du comportement. Comment développe-t-on un problème de jeu ? Cette question sera abordée dans une version ultérieure des présents travaux. L’ensemble du cadre théorique que nous commençons ici à élaborer sert à plusieurs escients : évaluer la pertinence d’interventions de prévention et de traitement, identifier des cibles de prévention, évaluer les programmes de traitement, développer des politiques publiques, identifier des pistes de recherche, pour ne nommer que ceux-là. No one is born a gambler. How do people get drawn into this activity? Three interrelated factors play a role in triggering this behavior. First, people must have the physical and mental capacity to engage in such activity. Second, they must have the opportunity to do so. In other words, games of chance must be available, and the person must have something to wager (usually money) and the time to gamble. Third, they must hold sufficiently positive views on gambling: the activity must appear to be legitimate and gamblers must have reason to believe in and hope for a benefit. While these three factors must all be present for a person to gamble, their presence alone is not enough to instigate the behavior. Why do people continue to gamble? These same factors operate in essentially the same way. To these we must add the intrinsically habit-forming nature of games of chance. How do gambling problems develop? This question will be addressed in a later version of this study. The theoretical framework that we are beginning to develop will serve a number of purposes, including assessing the relevance of prevention and treatment initiatives, pinpointing prevention targets, evaluating treatment programs, developing public policy, and identifying areas of research.

Abstract: Persönliche Beziehungen im Rahmen des beruflichen Netzwerkes begünstigten vor sechs Jahren den Start der Kooperation von vier ambulanten Suchtfachstellen der Kantone Aargau, Bern und Zürich. Ausgangspunkt war die gemeinsame Zielsetzung, im Rahmen eines Forschungsprojekts die mittel- und langfristige Wirksamkeit ihrer ambulanten Alkoholbehandlung zu messen. Verbindlichkeit, Dialogfähigkeit sowie Aufbau und Stärkung des gegenseitigen Vertrauens erweisen sich als die entscheidenden Erfolgsfaktoren.
