Abstract |
Ce document présente les résultats préliminaires du volet quantitatif du monitorage évaluatif du Programme expérimental sur le jeu pathologique. Il prend appui sur des données recueillies, de mai 2001 à octobre 2002, dans les 23 organismes de traitement du jeu pathologique choisis dans quatre sites-pilotes : (1 Montréal, Laval et Montérégie, (2 Québec et Chaudière-Appalaches, (3 Outaouais, (4 Bas-St-Laurent, Gaspésie et les Iles-de-la-Madeleine. Un total de 2030 dossiers ont été transmis à l’INSPQ pour cette période. Une partie de ces dossiers ont été complétés dès les débuts de la mise en oeuvre des services avant que nous ne proposions notre démarche de collecte de données. Il y a donc plusieurs données manquantes. Il ressort de l’analyse de ces données que 68 % des personnes inscrites au programme de traitement sont de sexe masculin et près des deux tiers sont âgés entre 35 et 54 ans. Trois personnes sur quatre possèdent l’équivalent d’un diplôme secondaire ou moins et 71 % des joueurs inscrits occupent un emploi. Les jeux auxquels s’adonnent majoritairement les joueurs en traitement sont les appareils électroniques de jeu (AÉJ). L’utilisation des outils diagnostiques du jeu pathologique (le SOGS et le DSM-IV) montre que le score moyen des participants est près de 12 sur 20 au SOGS et de 7 sur 10 au DSM-IV,ce qui dans les deux cas témoignent d’un problème de jeu pathologique. Par ailleurs, la proportion des joueurs qui souffrent d’un problème de jeu pathologique à l’arrivée, selon leur score au DSM-IV, est de 91 %. Ce qui signifierait que, 9 % de la clientèle inscrite serait une clientèle non visée par le programme. Les données pouvant expliquer ce constat sont insuffisantes pour l’instant. Des hypothèses peuvent toutefois être formulées. L’utilisation de ce test n’est pas homogène : il est utilisé à divers moment de la thérapie (à l’arrivée ou après quelques rencontres, par exemple), il est administré de façon très variée (le client peut, par exemple, le remplir par lui-même dans la salle d’attente, à la maison, ou encore avec le thérapeute lors d’une séance) et l’interprétation que les cliniciens en font diffère. Ces disparités dans l’utilisation peuvent influencer le score obtenu. Une différence notable est observée entre le taux de complétion du traitement par les joueurs inscrits aux organismes qui offrent des services à l’externe (33 %) et ceux qui offrent des services à l’interne (95 %). Des 66 % qui abandonnent le traitement en externe, le tiers le font en le signifiant à leur thérapeute. Ces données corroborent les constats déjà publiés dans la littérature sur la difficulté de maintenir la fréquentation et la motivation des joueurs en traitement externe parce qu’ils ne sont pas isolés de leur environnement d’obligations et de risques. Il faudra se pencher davantage sur les motifs d’abandon des clients en externe puisqu’une majorité de la clientèle s’y retrouve. Il faudra aussi questionner ce que le client en retire puisque la durée moyenne de traitement équivaut à plus de la moitié (environ 65 jours)de celle d’un traitement complété et le nombre moyen de rencontres de thérapie à plus du quart (de 5 à 6 rencontres). Un abandon n’est pas forcément un échec pour le programme si on tient compte du fait que l’ancrage dans un processus de traitement peut nécessiter pour certains quelques tentatives. This document presents the preliminary results of the quantitative component of monitoring and assessment of the Québec Experimental for pathological Gambling. It draws on data gathered from May 2001 to October 2002 in 23 pathological gambling treatment facilities selected in four pilot regions: (1) Montréal, Laval, and Montérégie, (2) Québec and Chaudière-Appalaches, (3) Outaouais, (4) Bas-St-Laurent and Gaspésie–Îles-de-la-Madeleine. A total of 2,030 files were sent to the INSPQ for this period, but since some had already been wound up before we began our information gathering, certain data is missing. Analysis of this data shows that 68% of treatment program participants were male, and nearly two-thirds were 35 to 54 years old. Three out of four participants had the equivalent of a high school diploma or less, and 71% of gamblers in the program were employed. The games in which the majority of gamblers in treatment engaged were electronic gaming devices (EGDs). Pathological gambling diagnostic tools (SOGS and DSM-IV) show that the average participant scored nearly 12 out of 20 on the SOGS and 7 out of 10 on the DSM-IV, which in both cases indicates a pathological gambling problem. In addition, 91% of program participants had a pathological gambling problem at the time of arrival, according to their DSM-IV score. This suggests that 9% of the registered clientele was not the clientele targeted by the program. However, this interpretation must be treated with caution due to a problem with the consistency of test conditions. Additional data and analyses will be necessary to support this conclusion. A marked difference was observed between the treatment completion rate of gamblers registered with organizations offering outpatient services (33%) and those offering inpatient services (95%). Of the 66% of clients who dropped out of outpatient treatment, one-third notified their therapist. These figures support what is already mentioned in the literature regarding the difficulty of getting gamblers in outpatient treatment to continue attending and stay motivated because they are not separated from their daily responsibilities and risks. Further examination of the reasons for outpatient client dropout is also necessary, since most clients fall under this group. It is also important to investigate what clients gain, since the average length of treatment (approximately 65 days) is over half that of full treatment, and the average number of therapy sessions (5 to 6 sessions) is over one-fourth. Dropping out does not necessarily signify program failure, given that anchoring in a treatment process may require a number of attempts for some people. |
