  Abstract Im Bereich der abstinenzorientierten stationären Suchttherapie verfügt man über einiges Wissen zu den Charakteristika des Klientels und zum Behandlungserfolg. Hinsichtlich der Qualität der Behandlung bestehen hingegen weitgehende Informationsdefizite. Folgende Ziele sollen mit der Studie in diesem Bereich erreicht werden: Durchführung von Qualitätskontrolle, Qualitätssicherung und -verbesserung, Formulierung von Qualitätsstandards sowie Hilfestellung für verantwortliche Behörden zur Planung des Behandlungsangebotes. 1998 und 1999 nahmen 93 stationäre Therapieeinrichtungen an der Erhebung der Strukturqualität teil. Informationen zur Prozessqualität wurden in einer Piloterhebung bei 28 Institutionen gesammelt. Dabei wurden Daten zur Planung des Therapieprozesses, zur Behandlungszufriedenheit des Klientels sowie zum Burnout-Niveau des Teams erhoben. Die Erfassung der Ergebnisqualität fand in einem anderen FOS-Pilotprojekt statt (s. Nachbefragungsstudie FOS). Zur Verbesserung der Therapiequalität wurden den Behandlungsinstitutionen ihre eigenen Ergebnisse sowie die durchschnittlichen Ergebnisse der übrigen Einrichtungen zugestellt. Um das eigene, institutionsspezifische Qualitätsniveau zu erkennen, sollten die zusammengefassten Durchschnittswerte der anderen Therapieeinrichtungen im Sinne eines Benchmarking Orientierungshilfen und Vergleichsmöglichkeiten bieten. Ein weiteres Ziel dieses Projektes ist es, in einem länger dauernden Prozess empirisch und normative begründete Standards für den Bereich der stationären Therapieangebote für Drogenabhängige zu entwickeln. Wichtige Elemente in diesem Ablauf sind die Unterstützung durch eine Expertengruppe und die Diskussion des Stands der Standards am FOS-Symposium vom 9. Dezember 1999 mit den anwesenden Praktikern und Praktikerinnen. Seit Anfang 1999 arbeitet das Bundesamt für Gesundheit an einem umfassenden und längerfristig angelegten Qualitätsprogramm mit dem Namen QuaTheDA. Mit diesem soll die Qualität der stationären Suchthilfe gesichert und verbessert werden. QuaTheDA bezieht sich explizit auf die vorliegende Studie und die beiden Projekte werden miteinander koordiniert.

  Abstract Using a sample of 614 youths, this study examined differences and similarities between adolescents at risk (age range 11-20 years), and their integration in school or vocational training. We found that mood states (depression, anxiety), secure self (self-esteem and self-efficacy), relationship with parents, delinquency and substance use (alcohol, tobacco and cannabis) were variables which largely influenced social integration (school, work) for youths with poor sociodemographic backgrounds. An important variable we introduced here was sociodemographic background, which investigated the social situation of a youth. We distinguished two different groups (good vs. poor sociodemographic background). There were important differences between both groups. Youths with a poor sociodemographic background scored higher in suicidality and health complaints. The most striking differences were in substance use and delinquency. The structural equation model showed that poor mood was related to substance use at the start. Important paths were found in mood states over time, as for substance use, but interactions between both variables were small. We ran different models (poor vs. good social background, Swiss vs. migrant youths), and concluded that secure self and good family relations protected against poor mood and substance use and both variables indirectly influenced integration 2-3 years later.

  Abstract There is a growing recognition around the world that the prohibition of drugs is a counterproductive failure. However, a major barrier to drug law reform has been a widespread fear of the unknown – just what could a post-prohibition regime look like? For the first time, ‘After the War on Drugs: Blueprint for Regulation’ answers that question by proposing specific models of regulation for each main type and preparation of prohibited drug, coupled with the principles and rationale for doing so. We demonstrate that moving to the legal regulation of drugs is not an unthinkable, politically impossible step in the dark, but a sensible, pragmatic approach to control drug production, supply and use.
