  Abstract This international report is the fourth from the Health Behaviour in School-aged Children (HBSC) study, a WHO collaborative cross-national study, and the most comprehensive. It presents the key findings on patterns of health among young people aged 11, 13 and 15 years in 41 countries and regions across the WHO European Region and North America in 2005/2006. Its theme is health inequalities: quantifying the gender, age, geographic and socioeconomic dimensions of health differentials. Its aim is to highlight where these inequalities exist, to inform and influence policy and practice and to help improve health for all young people. The report clearly shows that, while the health and well-being of many young people give cause for celebration, sizeable minorities are experiencing real and worrying problems related to overweight and obesity, self-esteem, life satisfaction, substance misuse and bullying. The report provides reliable data that health systems in Member States can use to support and encourage sectors such as education, social inclusion and housing, to achieve their primary goals and, in so doing, benefit young people’s health. Policy-makers and professionals in the participating countries should listen closely to the voices of their young people and ensure that these drive their efforts to put in place the circumstances – social, economic, health and educational – within which young people can thrive and prosper.

  Abstract Cet article traite des problèmes liés à la coordination de l’action publique dans des environnements complexes et fragmentés, à l’exemple de la politique de la drogue dans les principales agglomérations suisses. Deux lignes de conflit principales sont identifiées : les tensions résultant de l’implication de services administratifs et de groupes professionnels aux systèmes de valeurs différents ; et celles entre communes concernant la prise en charge des coûts de la politique de la drogue. Puis, sur la base d’entretiens semi-directifs, l’article explore dans quelle mesure des mécanismes de coordination remédient à ces conflits. Enfin, il met en évidence les problèmes de légitimation démocratique induits par cette forme de conduite de l’action publique. This article discusses problems related to the co-ordination of policymaking within complex and fragmented environments by taking as an example drug policy as it is practised in major Swiss cities. Two major lines of conflict are identified : a first set of tensions arises from the fact that administrative agencies and professional groups implicated do not share a common belief ; another set results from conflicts between communities concerning the costs and charges induced by drug-related measures. Based on semi-open-ended interviews, the article then explores to what extent co-ordination mechanisms may help to reduce these tensions. Finally, the authors examine problems of democratic legitimisation that may arise from such negotiated public action.

  Abstract BACKGROUND: Acute alcohol consumption has been reported to be an important risk factor for injury, but clear scientific evidence on issues such as injury type is not available. The present study aims to improve the knowledge of the importance of alcohol consumption as an injury determinant with regards to two dimensions of the type of injury, namely the nature and the body region involved. METHODS: Risk relationships between two injury type components and acute alcohol use were estimated through multinomial and logistic regression models based on data from 7,529 patients-among whom 3,682 had injury diagnoses-gathered in a Swiss emergency department. RESULTS: Depending on the type of injury, between 31.1% and 48.7% of casualties report alcohol use before emergency department attendance. The multinomial regression models show that even low alcohol levels are consistently associated with nearly all natures of injury and body regions. A persistent dose-response effect between alcohol levels and risk associations was observed for almost all injury types. CONCLUSIONS: The results highlight the importance and consistency of the risk association between low and moderate levels of acute alcohol consumption and all types of injury. None of the body regions and natures of injury could pride on absence of association between alcohol and injury. Public health, prevention, and care implications are considered.
