| Abstract | Le présent document fait partie du rapport d’évaluation du programme expérimental sur le jeu pathologique. Il touche un des volets du monitorage qualitatif du programme qui est le processus d’adaptation du programme dans les premiers mois de son implantation. Il repose spécifiquement sur l’analyse d’une série d’entrevues réalisées de juillet à novembre 2001auprès des directeurs ou coordonnateurs cliniques des 23 organismes de traitement des quatre sites-pilotes choisis dans le programme provincial : (1) Montréal, Laval et Montérégie,(2) Québec et Chaudière-Appalaches, (3) Outaouais, (4) Bas-St-Laurent—Gaspésie-Îles-dela-Madeleine. Ces entrevues visaient principalement à voir comment ces instances réagissaient à la proposition de programme de traitement venant du Centre québécois d’excellence sur la prévention et le traitement du jeu (CQEPTJ) et à examiner ce qu’ils mettaient ou avaient l’intention de mettre en oeuvre sur le terrain, dans leur environnement et avec leur expérience spécifiques. L’analyse du matériel de ces entrevues, en comparaison avec le modèle du CQEPTJ, nous permet de voir comment le « programme implanté »précise, renouvelle, se démarque ou s’oppose à ce dernier. Elle permet finalement d’identifier les zones de robustesse et de faiblesse du programme, de porter un premier jugement sur sa vraisemblance, sa capacité de produire les résultats souhaités. La comparaison entre ce que les entrevues révèlent de l’implantation et ce que le CQEPTJ proposait, repose sur la démarche méthodologique d’appariement à un modèle. Les résultats sont présentés en fonction de quatre grands processus d’implantation du programme : la délimitation de la clientèle, la transition de la clientèle, l’actualisation du traitement, la consolidation du traitement. La délimitation de la clientèle, c’est-à-dire la définition de son éligibilité et par la suite sa sélection, est au centre de la dynamique entre l’offre et la demande de services. Elle a tendance à être plus systématique pour les organismes qui ont une forte pression de la demande, ont fait le choix d’une orientation de groupe pour le traitement et n’ont pas de points de services dispersés sur leur territoire. À l’encontre du programme proposé, mais pour répondre aux besoins prioritaires des clients, il se fait une sélection bien avant le processus formel d’évaluation prévu. Le processus de transition de la clientèle prend une place plus importante, imprévue à l’origine. Une clientèle à multiples problématiques très présente, une demande importante, la présence d’un réseau de services disponibles et la volonté de réduire les abandons,suggèrent à une majorité d’organismes la création de mécanismes de transition (rencontres et groupes d’accueil, d’orientation, d’engagement, de motivation, de soutien) qui cherchent à mieux préparer les clients au traitement. Le danger d’« écrémage » de la clientèle est toutefois soulevé. La réalisation du programme proposé, sur le plan du traitement proprement dit, est vécue de deux manières, soit comme l’utilisation d’un noyau de programme à améliorer, soit comme l’intégration d’un outil à un programme plus large. Dans les deux cas, l’ordonnancement du programme proposé est revu pour travailler au besoin les conséquences financières,familiales, conjugales et occupationnelles du jeu, ses dimensions émotives et l’urgence de faire face aux rechutes, avant de passer à l’aspect plus rationnel du volet cognitif des conceptions erronées sur le jeu. Le travail de consolidation des acquis du traitement, peu développé dans le programme proposé, en est à sa conceptualisation et est orienté par les besoins des clients. Il y a de nombreuses tentatives d’intégration des proches à différentes étapes du traitement. Dans une implantation faite d’expériences, d’initiatives et d’innovations, la question de savoir si le programme mis en oeuvre peut vraisemblablement produire les effets escomptés reste ouverte puisque les cibles et les résultats n’ont jamais fait l’objet d’une délibération, ni d’un consensus minimal entre les principaux intéressés (promoteurs, concepteurs, gestionnaires,intervenants et usagers du programme). Les premiers résultats d’implantation devraient être l’occasion d’avancer dans la précision de ces objectifs et des pratiques thérapeutiques qui les accompagnent. This document is part of the evaluation of the experimental program for pathological gambling. It touches on one aspect of the qualitative program monitoring, that of the program adaptation in the months following its implementation. It draws specifically on analysis of a series of interviews with the clinical directors or coordinators of the 23 treatment facilities in the four pilot regions chosen in the provincial program: (1) Montréal-Laval-Montérégie, (2) Québec–Chaudière-Appalaches, (3) Outaouais, (4) Bas-St-Laurent–Gaspésie–Îles-de-la-Madeleine. These interviews primarily sought to determine what these organizations felt about the treatment proposed by the Centre québécois d’excellence sur la prévention et le traitement du jeu (CQEPTJ) and what portions of it they had implemented or intended to implement in the field, in their community, and what were their specific experience with the amended program. Comparing the content of these interviews with the CQEPTJ model shows us how the implemented program elaborates on, renews, differs from, or contradicts the original model. It also allows us to identify the program’s strengths and weaknesses and make a judgment as to its plausibility, or ability to produce the desired results. The interviews were examined with classical thematic content analysis and classification techniques, and more recent structural and systemic analytic methodologies. The content of these interviews on program implementation was compared to the CQEPTJ proposal using the methodological approach of pattern matching. The results are presented as a function of the four major program establishment processes: participants delimitation, participants’ transition, treatment implementation, and treatment consolidation. Participants delimitation, i.e., determining client eligibility and selection, is central to the dynamic between service supply and demand. It tends to be more systematic in organizations with high demand, with a group treatment orientation, and with no additional points of services throughout their areas. Contrary to the proposed program, but in order to meet the primary needs of clients, selection occurs well before the planned formal evaluation process. The client transition process is turning out to be broader in scope than initially expected. Given frequent comorbidity among the clientele, high demand, the presence of a network of available services, and a desire to decrease client dropout, most organizations tend to create transition mechanisms (welcome, orientation, commitment, motivation, and support meetings and groups) in an effort to better prepare clients for treatment. The danger of client «skimming» has, however, been raised. Implementation of the proposed program in terms of treatment activities is viewed in two ways, i.e., as a core program to use and improve or as one of a number of tools in a more including program. In both cases, the proposed program is reworked as needed to address the financial, family, marital, and occupational consequences of gambling, its emotional dimensions, and the urgent need to address relapses before moving on to the rational and cognitive component of correcting erroneous thoughts in gambling. The work of consolidating treatment progress—little discussed in the proposed program—is at the conceptualization stage and is mostly determined by client needs. Numerous attempts are made to integrate family and friends at various stages of treatment. Where program implementation is based on experience, initiative, and innovation, the question of whether it can truly produce the expected impact still remains open to discussion, since the targets and results have never been submitted to debate and reached a minimal consensus among the primary stakeholders (program promoters, designers, managers, administrators, and users). Early program results should provide an opportunity to further refine these objectives and the associated therapeutic practices. | ||||

Seine manchmal irritierend wohlwollende Sicht auf eine hoffnungslose Verfallsgeschichte nimmt oft grotesk-komische Züge an. Sein Optimismus aber ist unerschütterlich. Er kann nicht anders, als die Exzesse seines Vaters als sorgfältig geplante Kapitel einer noch geheimen Erfolgsgeschichte zu verstehen. Wie sonst sollen es die beiden je zu einem Happy End bringen? Der Autor Dominik Bernet, 1969 in Basel geboren, schreibt Drehbücher für Kino und Fernsehen. Sein Roman- Erstling “Marmorera” wurde zum Bestseller und kam in der Verfilmung von Markus Fischer in die Schweizer Kinos. Bernet wurde mit Förderpreisen ausgezeichnet, zuletzt von der Fachstelle Kultur des Kantons Zürich und vom Fachausschuss Literatur BS/BL für seinen zweiten Roman “Der grosse Durst”. | Abstract | Bei dem Stichwort Sucht denkt man häufig als Erstes an Drogensucht, insbesondere an die Abhängigkeit von verbotenen Betäubungsmitteln. Dabei passiert es leicht, dass die Abhängigkeit von legalen Suchtmitteln, insbesondere von Alkohol und Medikamenten, übersehen wird. Eine besondere Herausforderung stellt dabei insbesondere die Sucht im Alter dar. Als Lösungsansatz stellen die Autorinnen eine Variation des Case Managements vor, das nach einer eingehenden Vertiefung in die Ausgangsproblematik hinsichtlich seiner Funktionen und Aufgaben betrachtet wird. Wie sich eine derartige Konzeption praktisch umsetzen lässt, wird abschließend anhand eines Fallbeispiels vorgestellt. Darüber hinaus wird ein Weg entwickelt, wie Abhängige in Kooperation mit ihrem sozialen Umfeld agieren können. | ||||

| Abstract | Ein beträchtlicher Teil der Klientel der Suchthilfe in der Schweiz weist einen Migrationshintergrund auf, was Institutionen und Fachpersonen vor besondere Herausforderungen stellen kann. Dazu gehören Zugangsschwierigkeiten und Kommunikationsprobleme sowie divergierende Vorstellungen von Sucht und Heilung. Dabei sind für eine Suchtproblematik weniger die Migration an sich oder ein bestimmter ethnischer Hintergrund ausschlaggebend als damit verbundene sozio-ökonomische Benachteiligungen.
Transkulturelle Kompetenz und eine transkulturelle Organisationsentwicklung ermöglichen es Fachpersonen und Institutionen, alle diese Aspekte einbeziehend ganzheitlich auf die individuelle Situation einer Klientin oder eines Klienten einzugehen. Der vorliegende Beitrag fokussiert auf migrationsspezifische Aspekte im engeren Sinn, um den Stand der migrationsgerechten Suchtarbeit in der Schweiz aufzuzeigen. Diese wurde seit den 1990er Jahren durch verschiedene Initiativen des Bundesamtes für Gesundheit gefördert. Eine Auswertung der Infodrog-Datenbank der Suchthilfeangebote in der Schweiz zeigt, dass heute rund 10 % der Einrichtungen explizit ein migrationsspezifisches Angebot ausweisen. Von diesen verfügen knapp 9 % über ein eigentliches Migrationsmainstreaming, die übrigen beschränken sich auf einzelne Aspekte wie Gewährleistung der sprachlichen Verständigung, Vernetzung mit spezialisierten Einrichtungen oder Einbezug von Migrationsaspekten in Anamnese und Therapie. Die Auswertung zeigt, dass eine weitere Sensibilisierung nötig ist, verbunden mit einer Auweitung auf andere Aspekte der Diversität. |
