Abstract |
Le monitoring de la problématique du cannabis en Suisse constitue un ensemble de travaux qui permettent le suivi de la situation au niveau national et qui sont mis en oeuvre par un consortium d’institutions de recherche. Ce monitoring comprend l’étude présentée dans ce rapport, l’étude sentinelle. Celle-ci s’intéresse à l’évolution de la situation en matière de cannabis ainsi qu’à la gestion de cette situation au niveau local. Il s’agit de répondre aux questions suivantes : – quelle est la situation en matière de consommation de cannabis et de marché et quelle est son évolution ? – quels sont les principaux problèmes rencontrés sur le terrain ? – quelles sont les mesures et interventions qui ont été développées dans ce domaine ? Pour y répondre, on a choisi de suivre la situation dans quatre cantons suisses dits “sentinelle” (St-Gall, Tessin, Vaud, Zurich). Les critères de choix de ces cantons font appel à leur taille, au rapport ville/campagne et à la présence de frontière avec des états voisins, à la langue, au type de politique drogue pratiqué. Dans chaque canton on a constitué des panels d’experts formés par des profes-sionnels de terrain dans trois domaines différents (santé et social, école, police et justice). Leurs observations ainsi que les données cantonales disponibles sont récoltées et discutées lors d’un workshop et analysées sur plusieurs années. |

Abstract |
Die Kohortenstudie (supra-f ) ist die Weiterführung des supra-f Projektes. Das supra-f Projekt besteht aus differenzierten Unterstützungsangeboten für Jugendliche in Risikosituationen. Unter Risikosituationen sind weitgehend externale, aber auch internale Auffälligkeiten zu verstehen. Das Projekt startete 1999 mit Beteiligung von zwölf Zentren. Die Begleitevaluation des Projektes (Hüsler et al., 2006) zeigte, dass die Interventionen weitgehend strukturgebender Natur waren und in drei unterschiedlichen Hilfsangeboten zusammengefasst werden können. In den A-Zentren sind eher unauffällige Jugendliche der Altersgruppen 11 bis 15 Jahre zu finden, die noch in die Schule gehen. Die A-Zentren bieten deshalb punktuelle Unterstützung im Rahmen von 2 bis 4 Stunden pro Woche an. Die B-Zentren bieten Unterstützung für Jugendliche an, die nicht mehr in die Schule gehen und bei denen die Gefahr einer sozialen Desintegration besteht. Deshalb bieten die B-Zentren eine weit grössere Unterstützung im Rahmen von bis zu 20 Stunden pro Woche an, wobei es sich um Tagesstrukturen handelt. Die C-Zentren sind eine erweiterte Form der B-Zentren und bieten berufsintegrierende Massnahmen an, die über 20 Stunden pro Woche hinausgehen. Dieser Zentrumstyp ist ähnlich den Motivationssemestern des SECO, die in der ganzen Schweiz Verbreitung gefunden haben. Nach Abschluss des Projektes im Jahr 2005 wurden die Jugendlichen im Jahr 2006 in einer ersten Kohortenbefragung zusätzlich untersucht. Sie soll Aufschluss über die berufliche und soziale Integration geben. Die Haltequote ist mit fast 70% erstaunlich hoch für eine Population, deren wesentliches Problem in der Gefahr einer Desintegration besteht. Da die Kohorte auch eine Vergleichsgruppe ohne supra-f Intervention mitführte, können die Resultate verglichen werden. Im Jahr 2005 wurde der Begriff der «sozialen Ausgangslage » eingeführt, der sich auf soziodemografische Daten stützte, mit dem Ziel, die soziale Geschichte eines Jugendlichen zu erfassen (Hüsler & Werlen, 2005). Die soziale Ausgangslage steht in einem wesentlichen Zusammenhang mit praktisch allen untersuchten Variablen (Befinden, Substanzkonsum, Integration). Eine gute soziale Ausgangslage geht mit geringen Befindensbeeinträchtigungen, einem tiefen Substanzkonsum sowie einer guten sozialen und beruflichen Integration einher. Das Umgekehrte ist der Fall bei einer schlechten sozialen Ausgangslage. Eines der wesentlichsten Resultate und aus gesellschaftlicher Sicht sehr bedeutsam dürfte sein, dass auch Jugendliche, die mit starken Desintegrationsrisiken behaftet sind, erfolgreich integriert werden können. |

Abstract |
Ce rapport, établi à la demande de l’Office fédéral de la Santé publique, vise à étudier, à partir des textes législatifs et de la littérature spécialisée, les choix effectués par les pays de l’Union européenne en matière d’incrimination de la consommation de stupéfiants et des actes préparatoires de la consommation. Sept pays ont été sélectionnés (D, E, F, UK, I, NL, S), représentatifs des différences législatives existant au sein de l’UE en matière de consommation de drogues illicites. Le but de l’étude est de présenter les options qui s’offrent à la Suisse dans le cadre d’une révision de la Loi fédérale sur les stupéfiants, à la lumière de la latitude permise par le contexte international et des conclusions qu’une comparaison des législations mises en œuvre dans d’autres pays européens permet de tirer. |

Abstract |
Background and Aims: Substance users are at high risk of viral (HBV and HCV) and alcoholic liver disease leading in some to progressive fibrosis and cirrhosis. Users in rehabilitation programs often lack information about health risks related to substance use and ways for prevention and treatment and often postpone medical workup and treatment. The aims were: 1. To improve knowledge and awareness about the epidemiology, mode of acquisition, natural history, diagnosis and treatment of chronic viral hepatitis B and C in persons participating in substance use rehabilitation programs in southern Switzerland. 2. To identify users with liver disease and fibrosis or cirrhosis using ALT and Fibroscan testing, and 3. To offer personalized counseling on diagnostic workup and treatment for persons with significant fibrosis or cirrhosis in order to prevent progression to cirrhosis and its complications. Methods: Substance use rehabilitation programs were contacted and offered an educational event, held by an MD, with information on epidemiology, mode of acquisition, natural history, diagnosis and treatment of chronic alcoholic and viral hepatitis B and C. Participants interested were given the possibility to undergo ALTtesting, Fibroscan® testing as well a consultation with a hepatologist, free of charge. Results: Eleven Institutions were involved in the program, 7 offering residential and 4 daytime rehabilitation programs. A total of 136 users and 22 health care workers attended the educational sessions. 82 of whom (60.3%) participated in the ALT/Fibroscan screening program. The mean age was 43.9 years (65 male and 17female users). 45% had abnormal ALT levels. The mean was 89.6 IU/ml. Only 17% had normal liver elasticity, 83% had abnormal values compatible with fibrosis assessed by Fibroscan (>4.0 KPa). 21.9% had at least moderate (F2-F4) fibrosis (>8.0 KPa) and 10.9% had values indicating the presence of cirrhosis (>16 KPa, F4).The running costs in terms of personnel and materials of this pilot program were slightly more than 100 Swiss Francs per participant. Conclusions: The results confirm a high burden of liver disease in substance users in rehabilitation programs in our region. The interest and adherence to the program was excellent with 60.3% of those participating in the educational event undergoing voluntary ALT and Fibroscan® testing followed by counseling. Significant liver disease was found in more that 4/5 of the participants. More than 10% had results compatible with cirrhosis. Similar programs can be adapted to other high-risk settings for liver disease at reasonably low costs. |
