Abstract: This report, which provides a knowledge base for the development of methadone maintenance treatment clinical guidelines in Switzerland, reviews the world-wide literature (except for Switzerland, which is the subject of a separate investigation) on such treatment and existing guidelines. Articles on methadone maintenance treatment and concurring treatment modalities are discussed in detail, as well as existing knowledge about specific populations, such as pregnant addicts and addicts with HIV or tuberculosis. Research on prognostic factors to patients’ responses to treatment and perceptions of opiate dependent persons are also reviewed to ascertain their value to practitioners in managing methadone maintenance treatment and in assessing the best possible treatment. A final chapter describes other substitution treatments in comparison to methadone maintenance and synthesizes the information of the review into a structured information source for thinking about guideline development.

Abstract: Die Kardiotoxizität von Methadon durch Verlängerung der QTc Zeit im EKG wird in den letzten Jahren vermehrt diskutiert. Die Zusammenhänge zwischen Methadondosierung, QTc Verlängerung im EKG und dem damit assoziierten Risiko von lebensbedrohlichen Herzrhythmusstörungen sind jedoch noch ungenügend geklärt und die Literatur dazu ist widersprüchlich. In dieser Ausgabe des Evalinfo geben wir eine aktuelle Literaturübersicht zum Thema QTc Verlängerung durch Methadon und stellen die Ergebnisse einer Auswertung von Behandlungsdaten der ARUD Zürich vor. Schliesslich sollen praktische Empfehlungen zur Handhabung von EKGs und möglicherweise auftretenden QTc Verlängerungen in substitutionsgestützter Behandlung gegeben werden. Die Übersicht über die aktuelle Literatur und unsere Daten sprechen dafür, weiterhin in erster Linie bei hohen Methadondosen ein EKG zu veranlassen. Bei zusätzlichen Risikofaktoren sollte man auch bei Beginn der Substitution und bei tiefen Methadondosen eine EKGAbleitung in Erwägung ziehen. Die Patienten sollten über ein mögliches Risiko aufgeklärt werden und allfällige Zeichen wie Synkopen, Schwindel und Palpitationen erkennen können.

Abstract: Bereits 1993 zeigten Hemmeter et al. eine verminderte Tonuserniedrigung der peripheren Muskulatur in der REM-Schlafphase unter Methadon-Razemat-Substitution. Dieser Befund ist der REM-Schlaf-Verhaltensstörung zuzuordnen; empfohlene Therapie ist die Gabe von Clonazepam am Abend. Viele Substituierte berichten, dass sie mit einer Benzodiazepinmedikation einerseits besser schlafen können und sich andererseits nicht so niedergeschlagen und inaktiv fühlen. Eine Benzodiazepinmedikation wurde bzw. wird oftmals als sog. Beigebrauch betrachtet und steht somit dem Therapieziel der Substitutionsbehandlung entgegen. Diese Sichtweise muss auf dem Hintergrund der Datenlage hinterfragt werden, allerdings ist eine prophylaktische Benzodiazepinmedikation nicht indiziert. Already 1963 Hemmeter et al. had shown a missing tension decrease of musculature in REM-sleep during d,l-methadone substitution. This fact is assigned to the REM-sleep-behavior-disorder. Recommended medication is clonazepam in the evening. Much substituted patients reports on a better sleep with benzodiazepines. Add to this they don’t feel so depressed and inactive. A benzodiazepine-use is often seen as illicit drug use, which stands in the way of the aim of methadone maintenance therapy. This view is to scrutinize in sight ofthe-facts. However a preventive benzodiazepine-medication is not indicated.
