Abstract: Le dispositif fribourgeois de prise en charge des personnes souffrant d’addiction dispose de prestations variées et adaptées à beaucoup de groupes cibles. Cependant, le Conseil d’Etat, conscient que des améliorations dans la prise en charge sont à apporter, a mis sur pied, en 2008, le projet de “Coordination de la prise en charge des personnes souffrant d’addiction”. Mandaté par la Direction de la santé et des affaires sociales (DSAS), le projet a été coordonné par un chef de projet rattaché au Service du médecin cantonal et piloté par un comité composé de chef-fe-s de service de la DSAS et d’une représentante de la Direction de la sécurité et de la justice (DSJ). Le résultat visé par le projet de coordination était la mise en place d’un dispositif cantonal de prise en charge des personnes souffrant d’addiction ayant les caractéristiques suivantes : – une offre de qualité adaptée aux besoins et problématiques actuels et démontrés, – une collaboration interinstitutionnelle et interdisciplinaire, – une chaîne thérapeutique centrée sur la personne souffrant d’addiction, – une utilisation optimale des ressources à disposition (efficacité et efficience), – un souci constant d’amélioration et d’adaptation (besoins/offres, fonctionnement, résultats). Un groupe de projet, composé de représentant-e-s d’institutions et services directement ou fortement impliqués dans la prise en charge de personnes souffrant d’addiction, a élaboré – à l’attention du Conseil d’Etat – des propositions de mesures à prendre pour atteindre les objectifs du projet. Le projet a été financé par le Fonds pour la lutte contre les toxicomanies. Il a pris en considération conjointement les problématiques des addictions à l’alcool et aux drogues illégales. Ce rapport présente les résultats et les propositions élaborées dans le cadre du projet et retenues par le Conseil d’Etat et est structuré de la manière suivante : – Présentation du projet, de son cadre et de ses objectifs (chapitre 2) – Généralités (addictions, épidémiologie, coûts sociaux, dispositif fribourgeois actuel) (chapitre 3) – Besoins (chapitre 4) – Résultats et propositions (chapitres 5) Les résultats et les propositions sont présentés par domaines, à savoir : – Prestations (chapitre 5.1) – Indication et case management (chapitre 5.2) – Organisation de la fondation le Tremplin, de la fondation le Torry et de l’association le Radeau (chapitre 5.3) – Structure de coordination et de pilotage (chapitre 5.4) La fin du projet de coordination représente la fin d’une première étape. La seconde étape consistera dans la réalisation des propositions présentées.

Jede «Heilung» ist unter diesen theoretischen Voraussetzungen eine «Selbst-Heilung» – nur einfach unter anderen Voraussetzungen.

Abstract: Fragestellung: Bisher ist wenig darüber bekannt, welche internen bzw. externen Effekte eine erfolgreiche Zertifizierung hat. Hier setzt eine Studie an, die Anfang 2007 von der Deutschen Gesellschaft für Qualitätsmanagement in der Suchttherapie e. v., Kassel (de-Qus) in Zusammenarbeit mit Prof. Dr. Aloys Prinz (Gesundheitsökonom am Fachbereich Wirtschaftswissenschaften an der WWU Münster) durchgeführt wurde. Ziel der Studie war es, die internen und externen Auswirkungen einer Zertifizierung zu untersuchen. Methodik: Zertifizierte deQus-Mitglieder wurden anhand eines Fragebogens nach ihren anfänglichen Erwartungen bzgl. interner und externer Effekte einer Zertifizierung und nach den tatsächlich erzielten Wirkungen gefragt. Ergebnisse: Die Ziele der Einrichtungen, mit der Zertifizierung einen insbesondere von den Leistungsträgern anerkannten Qualitätsnachweis zu erbringen, wurden nicht erreicht. Die intern realisierte Qualitätsentwicklung überstieg jedoch die ursprünglichen Erwartungen. Schlussfolgerungen: Die gesetzliche Zertifizierungspflicht und die diesbezüglichen Vorgaben der Bundesarbeitsgemeinschaft für Rehabilitation (BAR) lassen erwarten, dass zukünftig eine Zertifizierung auch als Nachweis eines guten Qualitätsniveaus gelten wird. Aim: Because little was known about the internal and external effects of successful certification, this study aimed to determine what these effects are. Method: Certified members of the German Society of Quality Management in Addiction Therapy («deQus») were asked to complete a questionnaire on their initial expectations about the internal and external effects of obtaining certification and the success that they actually achieved. Results: The goal of the institutions to provide proof of quality to certification agencies was not achieved. However, the improvement in quality that they realized exceeded initial expectations. Conclusions: The legal obligation set by the German Federal Study Group for Rehabilitation (BAR) that institutions obtain certification has led to the expectation that in future certification will be accepted as proof of a high level of quality.
