Abstract |
Drug substitution. What role do methadone maintenance programmes or heroin-assisted treatment play in present-day drug policy? Can they aim for cure or are they merely palliative? To what extent is drug substitution therapy? These and other basic questions were considered by over 300 participants in the presentations and 19 workshops at the National Substitution Conference (NASUKO) held in Berne on 6 and 7 September 2007. |

Abstract |
Increasingly younger «binge drinkers» tend to be the focus of public attention. This problem therefore overshadows another addiction problem that is all too often overlooked: alcohol abuse in older people: in Switzerland, some 73,000 people over the age of 65 regularly drink excessive amounts of alcohol. And this figure looks set to rise steadily given the demographic trend – over-65s will in future account for about a third of the Swiss population. Both ethically and economically, continuing to downplay or blank out the «just a few drinks» attitude is no longer acceptable. |

Abstract |
Die immer jüngeren «Rauschtrinker » stehen im Fokus der öffentlichen Aufmerksamkeit. Im Schatten des Jugendkonsums steht jedoch ein anderes Suchtproblem, das zu oft vergessen wird: der Alkoholmissbrauch im Alter. In der Schweiz trinken rund 73 000 Menschen über 65 regelmässig zu viel Alkohol. In Anbetracht der demografischen Entwicklung – in Zukunft werden die über 65-Jährigen ca. ein Drittel der Schweizer Bevölkerung ausmachen – wird diese Zahl stetig steigen. Die «zwei, drei Gläschen in Ehren» weiter zu bagatellisieren und auszublenden, ist sowohl ethisch wie auch ökonomisch nicht mehr tragbar. |

Abstract |
L’ivresse ponctuelle qui touche les jeunes de plus en plus tôt dans la vie attire l’attention du grand public. Mais, dans l’ombre de cette consommation des jeunes, se tapit un autre problème de dépendance, trop souvent oublié: l’abus d’alcool chez les aînés. En Suisse, 73‘000 personnes de plus de 65 ans boivent régulièrement trop d’alcool. Compte tenu de l’évolution démographique – à l’avenir les plus de 65 ans vont représenter environ un tiers de la population suisse – ce chiffre est appelé à croître. Banaliser les «deux ou trois petits verres» et les occulter n’est tout simplement plus soutenable, tant du point de vue éthique que dans une optique économique. |

Abstract |
La définition du concept d’addiction apparaît de plus en plus difficile à cerner. Des toxicomanies à l’alcoolisme et au tabagisme, en passant par le jeu pathologique, les achats compulsifs, la sexualité et certains troubles du comportement… jusqu’à l’addiction, pourtant si peu virtuelle des cyberdépendants, on peut se demander quel est le point commun des addictions sans drogue et tout d’abord s’il existe. Ce questionnement est incontournable pour bâtir des stratégies de prévention et d’intervention médico-sociale adaptées. Il a également pour conséquence de penser les addictions en terme de conduite des sujets plutôt qu’une approche à partir des produits psychotropes. Ce thema de Toxibase propose d’entamer le débat à partir d’une vision conceptuelle des addictions sans drogue et d’une description nosographique et sociale de chacune d’entre elles. |

Abstract |
Les formes de pratiques sociales des jeux sont aujourd’hui tellement diversifiées et répandues que la figure du joueur pathologique émerge désormais comme une nouvelle maladie. Il existe des arguments très forts pour inclure celle-ci dans la notion d’addiction. Cet article dresse un tableau du jeu pathologique en rappelant sa place économique et sociale et son évolution historique. Le paradoxe de cette “toxicomanie sans drogue” est qu’il s’agit évidemment d’une source de profit considérable pour l’Etat qui contrôle le système des paris et des casinos. On estime que le jeu pathologique touche 2 à 3% des adultes et qu’il s’agit d’une problématique surtout masculine. La définition du jeu pathologique (DSM-IV) met en valeur une comorbidité importante (dépression, personnalités antisociales, usage de drogues et d’alcool, trouble des conduites alimentaires). Sur le plan psychanalytique (à partir de Freud, Bergler, Fenichel) le joueur rechercherait, par une forme d’auto-punition compulsive, à résoudre l’ambivalence envers le père et l’enjeu de l’intégration de la loi, démarche comparable aux autres formes de toxicomanies. Une revue des propositions thérapeutiques est réalisée : abstention à visée thérapeutique, psychothérapies individuelles, thérapies comportementales et cognitives, thérapies de groupe, chimiothérapie psychotrope, associations et groupes d’entraide. Cet article présente une bibliographie de 32 références. |

Abstract |
Aim: The injection of non-sterile methadone designed for oral consumption is associated with serious health risks. There is only a small number of studies on this topic, with divergent results. The main aim of the present study was to obtain data on the frequency of methadone injecting in a state out-patient clinic specialised in substance use disorders. Methods: Eighty patients in methadone maintenance treatment were interviewed with a short questionnaire. Mean age was 32 years, 76% were male, mean methadone dose was 55 mg. Results: Twenty-six patients (32%) indicated having injected methadone at least once in their life. Only four patients (5%) reported having injected methadone within the preceding month (mean dose 56 mg). All four had injected additional substances during this month. Conclusions: Frequency rates appeared low compared with other studies despite a generous take-away policy. The results suggest an association between methadone injecting and a more general tendency to inject substances. |
