Abstract |
La revue des acquisitions thérapeutiques s’est portée sur cinq aspects bio-psycho-sociaux : 1. Les rapports entre méthadone et cardiotoxicité. 2. L’introduction de la désensibilisation et du retraitement par les mouvements oculaires chez les patients toxico-dépendants. 3. L’apparition d’une possible pharmacothérapie spécifique du jeu excessif. 4. Une meilleure connaissance des rapports entre cannabis et psychose. 5. Résistances au traitement dans la relation médecin-malade. This year reviews on the addictions emphasizes five aspects, on a bio-psycho-social perspective: (1) The relationship between methadone and cardiotoxicity. (2) The introduction of Eye Movement Desensibilization and Reprocessing (EMDR). (3) The apparition of a possible specific pharmacotherapy for excessive gambling. (4) A better knowledge of the relationship between cannabis and psychoses. (5) Resistance to treatment in the doctor-patient relationship. |

Abstract |
Ziel: Prüfung, ob in Spülflüssigkeit aus gebrauchten Heroinspritzen von Hepatitis-C-positiven Drogenkonsumenten Hepatitis-C-Viruslast nachweisbar ist. Diskussion der Hypothese, ob durch gemeinsames Zubereiten von Drogen in Gruppen bei Einsatz gebrauchter Spritzen die Weitergabe von Hepatitis-C-Viren über „indirect sharing” möglich ist. Methode: Gebrauchte Spritzen von sechs Hepatitis-C-infizierten Drogenkonsumenten wurden nach der Heroininjektion mit Kochsalzlösung durchspült; die Spüllösung wurde mittels PCR-Technik auf Nachweisbarkeit von Hepatitis-C-Viruslast untersucht. Die Höhe der Hepatitis-C-Viruslast im Blut der Patienten und in der Spülflüssigkeit wurde verglichen. Teilnehmer: Die sechs Probanden sind Teilnehmer an einem Heroinverschreibungsprogramm in der Schweiz (Poliklinik Zokl2). Ergebnis: In allen Spülflüssigkeiten konnte eine beachtliche Hepatitis-C-Viruslast nachgewiesen werden (7530 bis 269825 cop/ml). Schlussfolgerungen: Die gemessene Viruslast in gebrauchten Heroinspritzen ist ein weiteres Indiz dafür, dass das gemeinsame Zubereiten von Drogen mit Verwendung gebrauchter Spritzen ein Hepatitis-C-Infektionsrisiko darstellt. Detection of Hepatitis C Viral Load in Used Syringes: Aim: It is common for drugs to be prepared in groups and for used syringes to be involved in this process (indirect sharing). The aim was to detect viral load of rinsed syringes collected from hepatitis C positive heroin addicts. The following hypothesis is discussed: Is transmission of the hepatitis C virus possible by indirect sharing? Method: Used syringes from six hepatitis-C infected drug users were rinsed with 0.3 ml of NaCl solution shortly after injection. This fluid underwent PCR testing for hepatitis C viral load. The hepatitis C viral load in the blood of the study participants was also measured. Result: A significant hepatitis C viral load was detected in all syringe samples. The range of viral loads was between 7,530 and 269,825 cop/ml, while the blood samples yielded 1,503 to 23,067,000 cop/ml. Conclusions: The detection of Hepatitis C viral load in fluid samples of used syringes is a further indication that preparation of drugs in groups with used syringes can be a risk for hepatitis C transmission. |

Abstract |
La dépendance à l’alcool est une maladie chronique fréquente aux conséquences multiples qui nécessite un suivi à long terme. Le Service de la santé publique du canton de Vaud a décidé de mettre en place, sur l’ensemble du canton, un dispositif d’indication et de suivi alcoologique coordonné. Les buts d’un tel dispositif, exposé dans le présent article, sont de standardiser l’évaluation, la prise en charge et le suivi des patients alcoolo-dépendants, d’augmenter l’accessibilité aux soins en favorisant des approches thérapeutiques moins lourdes, et d’assurer une meilleure transition entre les structures résidentielles et le réseau ambulatoire. Ce dispositif vise à offrir une meilleure accessibilité aux soins spécialisés, à améliorer la rétention au traitement et une réduction des rechutes. |
