  Abstract Die Finanzlage der Invalidenversicherung (IV) ist äusserst kritisch. Die Gesamtausgaben sind von 1990 bis 2004 von 4,133 Mrd. auf 11,096 Mrd. Franken gestiegen. Dies entspricht einer Erhöhung von rund 168% in 14 Jahren. Gleichzeitig bleiben die Einnahmen immer weiter hinter den Ausgaben zurück. 2004 betrug der Fehlbetrag 1,6 Mrd. Franken, und das Kapitalkonto wies einen Negativsaldo – d.h. eine Schuld beim Ausgleichsfonds – von 6 Mrd. Franken aus. Bezogen auf das Bruttoinlandprodukt (BIP) der Schweiz mag der Anteil der IV bescheiden aussehen. Doch der Schein trügt, weil in den Sozialversicherungen die Dynamik von primärer Bedeutung ist. So wuchs das BIP von 1990 bis 2002 um 31,5%, also rund fünfmal weniger stark als die Ausgaben der IV. Die Gesamtausgaben für die Sozialversicherungen (bis 2003) stiegen um 85,5% an, also nur gerade halb so stark.

  Title Sexual uses of alcohol and drugs and the associated health risks : a cross sectional study of young people in nine European cities Type Journal Article

  Abstract Background: Young people in European countries are experiencing high levels of alcohol and drug use and escalating levels of sexually transmitted infections. Individually these represent major public health priorities. Understanding of the association between sex and substance use, and specifically the strategic roles for which young people utilise substances to facilitate sexual activity, remains limited. Methods: Respondent driven sampling methodology was used in nine European cities to survey 1,341 16-35 year olds representing youth and younger adults who routinely engage in nightlife. Participants self-completed questionnaires, designed to gather demographic, social, and behavioural data on historic and current substance use and sexual behaviour. Results: Respondents reported strategic use of specific substances for different sexual purposes. Substances differed significantly in the purposes for which each was deployed (e.g. 28.6% of alcohol users use it to facilitate sexual encounters; 26.2% of cocaine users use it to prolong sex) with user demographics also relating to levels of sexual use (e.g. higher levels of: ecstasy use by males to prolong sex; cocaine use by single individuals to enhance sensation and arousal). Associations between substance use and sex started at a young age, with alcohol, cannabis, cocaine or ecstasy use before age 16 all being associated with having had sex before the age of 16 (odds ratios, 3.47, 4.19, 5.73, 9.35 respectively). However, sexes differed and substance use under 16 years was associated with a proportionately greater increase in early sex amongst girls. Respondents’ current drug use was associated with having multiple sexual partners. Thus, for instance, regular cocaine users (c.f. never users) were over five times more likely to have had five or more sexual partners in the last 12 months or have paid for sex. Conclusion: An epidemic of recreational drug use and binge drinking exposes millions of young Europeans to routine consumption of substances which alter their sexual decisions and increase their chances of unsafe and regretted sex. For many, substance use has become an integral part of their strategic approach to sex, locking them into continued use. Tackling substances with both physiological and psychological links to sex requires approaching substance use and sexual behaviour in the same way that individuals experience them; as part of the same social process.

  Abstract La situation financière de l’assurance-invalidité (AI) est extrêmement critique. L’ensemble des dépenses est passé, de 1990 à 2004, de 4,133 milliards à 11,096 milliards de francs. Cela correspond à une augmentation de 168% en 14 ans. Parallèlement, les recettes ont progressé à une vitesse nettement inférieure, ce qui fait qu’en 2004 il manquait 1,6 milliard de francs. Le compte capital présentait en outre un solde négatif sous la forme d’une dette de 6 milliards de francs auprès du fonds de compensation. Par rapport à l’ensemble du produit intérieur brut (PIB) suisse, la part de l’AI peut sembler modeste, mais l’apparence est trompeuse. En effet, dans le domaine des assurances sociales, la dynamique est de première importance. C’est ainsi que le PIB, de 1990 à 2002, n’a augmenté que de 31,5%, soit cinq fois moins que les dépenses de l’AI: les dépenses pour les assurances sociales (jusqu’en 2003) ont progressé de leur côté de 85,5%, soit près de la moitié.

  Abstract Fragestellung: Welche neurobiologischen Erkenntnisse zu Ursachen und Folgen übermäßigen Alkoholkonsums bei Kindern und Jugendlichen sind gegenwärtig bekannt? Methodik: Überblick über einschlägige epidemiologische und experimentelle Studien. Ergebnisse: Eine Reihe neurobiologischer Faktoren beeinflusst das Trinkverhalten Jugendlicher und das Risiko für spätere Alkoholabhängigkeit. Sie sind größtenteils genetisch beeinflusst, entfalten ihr Risikopotential jedoch erst durch wechselseitige Interaktion mit Umwelteinflüssen. Bereits im Jugendalter können Alkoholfolgeschäden am Gehirn und seiner Funktion auftreten. Schlussfolgerungen: Das Vermeiden übermäßigen Alkoholkonsums im Jugendalter begünstigt den noch im Ablauf befindlichen Prozess der Gehirnreifung und verringert die Gefahr späterer Alkoholsucht. Aim: To describe what is currently known about the neurobiological causes and consequences of excessive alcohol consumption among children and adolescents. Methods: A review of the relevant epidemiological and experimental studies was carried out. Results: A number of neurobiological factors affect adolescent drinking behaviour and the risk of later developing alcohol dependence. These factors are primarily genetically determined, but the risk potential develops through reciprocal interactions with the environment. alcohol-related harmful effects on the brain and its functioning can begin during adolescence. Conclusions: Avoiding drinking excessively during adolescence will facilitate brain maturational processes and reduce the risk of later developing alcohol dependence.

  Abstract Im Sport zeichnen sich Entwicklungen im Umgang mit Medikamenten und psychoaktiven Substanzen ab, die vor allem Männer betreffen. Das zeigt auch die aktuelle Diskussion über den Einsatz von Dopingmitteln im Spitzensport. Deutsche Studien im Breiten- und Freizeitsport in Fitness-Studios und in der Bodybuilding-Szene belegen darüber hinaus, dass 20-40% der sportlich Aktiven Dopingmittel wie anabole-androgene Steroide und Stimulanzien nehmen. Das Verhältnis von Männern zu Frauen, die Dopingmittel nehmen, variiert zwischen 3:1 und 9:1. Diese Daten zeigen an, dass der Konsum von Medikamenten nicht mehr allein “typisch weiblich” sondern ebenso “typisch männlich” ist. In sports, developments are observed in the use of medications and of psychoactive substances by men. It is reflected in the ongoing discussion on doping in top sports events. Empirical studies in Germany in mass sports arenas, leisure time sports activities, in fitness-clubs and in the scene of body-builders show that between 20% and 40 % of those who are active in sports use substances such as anabolic-androgenic steroids and stimulants. The relationship between men and women who use such substances varies between 3:1 and 9:1. The data show that the use of medications is not only “typical female” but as weil “typical male”.

  Abstract Nach innovativen Ansätzen in den Neunzigerjahren droht die schweizerische Drogenpolitik zu stagnieren. Die Abstimmungen über die Revision des Betäubungsmittelgesetzes und die Hanfinitiative haben die öffentliche Diskussion über das Drogenproblem zwar wieder etwas belebt. Das Interesse der Bevölkerung ist aber bei Weitem nicht mehr so gross wie damals, als vor allem die Städte mit offenen Drogenszenen konfrontiert waren. Für die Zukunft braucht es breiter gefasste Ansätze, bei denen die aus fachlicher Sicht willkürliche Grenze zwischen illegalen und legalen psychoaktiven Substanzen aufgehoben wird.
