Abstract |
Das Folienrauchen verbreitete sich in der Schweiz anfangs der 90er Jahre rasch, wobei allerdings die Zahl der Heroinkonsumierenden insgesamt stabil blieb. Mit ethnographischen Mitteln wird hier diese Konsumform des Heroins in ihrem historischen und sozialen Kontext dargestellt. Objektive Rahmenbedingungen wie der Afghanistankrieg geraten dabei ebenso ins Blickfeld wie der öffentliche Umgang mit der zu Beginn des Jahrzehntes einsetzenden wirtschaftlichen Rezession. In Interviews mit rund 50 Jugendlichen wird insbesondere nach unerwartet vielfältigen und widersprüchlichen Formen meist unbewussten Nutzens gefragt, den die physische und soziale Selbstgefährdung den Drogenkonsumierenden bietet. Etwa den oft beklagten “Beschaffungsstress” einmal als haltgebende “Tagesstruktur” zu sehen, vermittelt wichtige Einblicke in die Mechanismen süchtigen Drogenkonsums. Wichtige Anhaltspunkte ergeben auch sorgfältig analysierte Ausdrücke des Zürcher Gassenslangs. Wie in einem Zerrspiegel lassen sich auch auf dieser sprachlichen Ebene karikierte konsumgesellschaftliche Werte wiedererkennen. |

Abstract |
In diesem 47. Band der Reihe Studien zur qualitativen Drogenforschung und akzeptierenden Drogenarbeit untersucht die Autorin das Expertenwissen, auf welches Sozialarbeiter im Zuge ihres Handelns in der Praxis akzeptierender Drogenarbeit zurückgreifen. Dabei widmet sie sich der Frage, welche Formen von “Akzeptanz” in Einrichtungen akzeptierender Drogenarbeit von den dort tätigen Experten als angemessen betrachtet werden. Untersucht wird, inwieweit diese dabei auf in der Professionsliteratur vorhandenes theoretisches Wissen über “Akzeptanz” zurückgreifen, ob sie dieses im Rahmen ihrer alltäglichen Arbeitsanforderungen modifizieren oder gar verwerfen und ob auf Grund praktischer Erfahrungen neues Wissen über “Akzeptanz” entwickelt wird. Die Fragen werden auf Grundlage einer qualitativen Untersuchung geklärt. Im Rahmen dieser wird das in der Professionsliteratur vorhandene Wissen über “Akzeptanz” untersucht und eine Deutungsmusteranalyse in der Praxis akzeptierender Drogenarbeit durchgeführt, um dann die jeweiligen Ergebnisse gegenüberzustellen. Die Autorin zeigt, welche unterschiedlichen Ausprägungen das Deutungsmuster “Akzeptanz” als zentrales handlungsleitendes Wissen der Sozialarbeiter in der Praxis hat. Des Weiteren wird deutlich, dass einerseits in hohem Maße auf professionsliterarisch fixiertes Wissen zurückgegriffen wird, dieses andererseits aber durch typische Praxisanforderungen stellenweise modifiziert und ergänzt wird. |

Abstract |
Recent estimates suggest that, each year, over 1 million Europeans receive treatment for problems related to the use of illicit drugs (EMCDDA, 2010). Of these, more than half receive opioid substitution treatment for problems related to opioid drugs, primarily heroin. The current levels of provision of drug treatment services in the European Union are the result of an expansion of services that has taken place during the last two decades. For much of that time, services expanded against a background of a growing drug problem, and one in which economic conditions were largely favourable. Now, however, in an era of cuts in government expenditure, managing the costs of treatment and ensuring the highest quality and best outcomes for the lowest possible cost are priorities. While measuring and improving drug treatment outcomes have been relatively high on the research agenda in recent years, the cost of treatment has received far less research attention. Addressing this information gap is likely to have a number of benefi ts for both decision-makers and treatment providers. On the one hand, service providers need accurate information on the costs of service provision in order to plan the allocation of resources. On the other, decision-makers and funders can use such information as a means of cost control, for example, by comparing costs of similar services or those of alternative providers of similar services. Finally, as a part of a fuller economic analysis, information on service costs is needed to determine the cost-effectiveness of interventions and treatment programmes. The cost of drug treatment in Europe is looked at from two main angles in this Selected issue. First, it presents an overview of the main sources of drug treatment funding and the financing mechanisms that are employed in European countries. The available data are presented on who is paying for drug treatment, how funding is organised, and how funding is distributed among different treatment modalities. This is followed by an analysis of the unit costs of different types of drug treatment. Costs related to opioid substitution treatment including the costs of substitution medication for a number of European countries are presented here. This publication keeps a tight focus on the cost of drug treatment and does not attempt to undertake a broader economic analysis of the healthcare and social costs relating to illicit drugs. This Selected issue represents a fi rst attempt to provide a European overview of drug treatment costs, and individual countries are brought into focus to illustrate specifi c aspects of the analysis. This subject is both topical and methodologically challenging. The varying availability of information as well as the complexity of funding arrangements mean that, at best, only very incomplete estimates can be made of the costs of (or expenditure on) drug treatment in Europe. |

This Selected issue is not intended to present a systematic and exhaustive review either of all possible definitions of drug-related public nuisance or of all measures, provisions and policies aimed at reducing the problem in the European Union, nor does it aim to reflect precisely the situation regarding public nuisance in each of the countries concerned. Rather, it seeks to contribute to our understanding of the issues and problems related to a new – and somewhat still limited – area for intervention in Member States, candidate countries and Norway. This document aims to present the first EMCDDA qualitative insight into an emerging concern within drug policy debate, at both national and European levels. The alternatives to prison that may be offered to drug-using offenders cover a range of sanctions that may delay, avoid, replace or complement prison sentences for those drug users who have committed an offence normally sanctioned with imprisonment by national law. In this Selected issue, the focus is on those measures that have a drug-related treatment component. It will describe the political and legal background, the application and implementation, including common problems, and the effects of treatment as an alternative to imprisonment. Since the mid-1990s buprenorphine has increasingly become available in Europe as an alternative to methadone for the treatment of opiate dependence. In this Selected issue, the reasons why clinicians are attracted to this drug, as well as the costs and benefits of buprenorphine in comparison with other treatment options, are explored in detail, and, for the first time, the increasing popularity of buprenorphine for the treatment of opiate dependence in many European countries is documented. |
