Abstract |
We all have an interest in knowing which policies work in tackling problems associated with drug use. Many members of the public, and many politicians, believe that our drug policies are not working. But the debate about how we address the challenges of mind-altering drugs is polarised in a way not seen in most other policy areas. The UK Drug Policy Commission was established to address these problems in a different way. Its aim has been to show how independent scrutiny of evidence can produce both better results and more effective use of resources in drug policy and practice. Existing drug policies have struggled to limit the damage drug use can cause, and now new challenges are emerging. The rapid development of new drugs is changing drug markets too quickly for the traditional methods we use to control drugs to be effective. The economic crisis may be impacting on the nature of drug use and drug problems and, with fewer resources, the capacity of services to respond will be limited further. Added to that, the speed and scale at which services are being devolved to a local level may create increasing and unpredictable variations in the kind of services offered in different parts of the UK. In this report, UKDPC proposes a radical rethink of how we structure our response to drug problems. It provides an analysis of the evidence for how policies and interventions could be improved, with recommendations for policymakers and practitioners to address the new and established challenges associated with drug use. UKDPC aims to foster a fresh approach to drug policy: one in which evidence takes priority, creating light rather than heat in the debate on drugs, so that we can create an environment that works to reduce dependence on drugs, safeguards communities and delivers value for money. |

Abstract |
Le chapitre I du Rapport mondial sur les drogues de cette année donne un aperçu des tendances récentes et de la situation de la production, du trafic et de la consommation de drogues et des conséquences de cette consommation en termes de traitements et de maladies et de décès liés à la drogue. Le chapitre II présente d’un point de vue diachronique les caractéristiques du problème de la drogue, son évolution et les principaux facteurs qui le déterminent. Il débute par un examen des principales caractéristiques du problème actuel, suivi d’une présentation des changements observés au cours des dernières décennies, puis s’achève par une analyse des facteurs qui ont influé sur son évolution et un bref aperçu des tendances futures probables. |

Abstract |
Initianten und Befürworter der Volksinitiative “Schutz vor Passivrauchen” haben das Abstimmungsresultat mit grosser Enttäuschung aufgenommen. Das unerwartet deutliche Nein des Stimmvolks überraschte, zumal nach ersten Meinungsumfragen eine Mehrheit der Befragten eine Unterstützung signalisiert hatte. Bedauerlich ist vor allem auch, dass das Resultat vom vergangenen 23. September Bestrebungen in den Kantonen für einen umfassenderen Schutz hemmen dürfte. Der Abstimmungskampf war mit Emotionen und irreführenden Aussagen geführt worden und sorgte wohl mitunter für Verunsicherung des Stimmvolks, das sich eben erst ans neue Bundesgesetz gewöhnt hatte. Nach heutigem Bundesrecht sind Raucherbeizen und bediente Fumoirs zulässig, sofern die Kantone keine strengere Regelung vorsehen. Die Initiative wollte Dritte noch umfassender als heute vor Tabakrauch schützen. Konkret hätte namentlich das Personal in der Gastronomie sowie die Gäste in Gastronomiebetrieben besser geschützt werden sollen. Sucht Schweiz sieht eine wichtige Chance für den Gesundheitsschutz verpasst, denn nach wie vor sind zu viele Arbeitnehmende dem schädlichen Tabakrauch während Stunden ausgesetzt. Gleich zwei aktuelle Studien geben Aufschluss über den Alkoholkonsum älterer Menschen. Während der mediale Fokus seit langem weit mehr auf dem problematischen Konsum der Jugendlichen und jungen Erwachsenen liegt, wurde den Trinkgewohnheiten der älteren Menschen bislang weniger Aufmerksamkeit geschenkt. Das Thema verdient einen breiteren Diskurs. Inwieweit die beiden neuen Studien von Sucht Schweiz und dem Institut für Sucht- und Gesundheitsforschung (ISGF) die öffentliche Debatte fortan anregen, ist noch kaum abzuschätzen. Fakt ist: Die Zeichen eines Alkoholmissbrauchs oder einer Abhängigkeit bleiben oft unerkannt oder sie werden auf die Einnahme von Medikamenten oder eine Demenz zurückgeführt. Depressionen oder Schlaflosigkeit schreibt das Umfeld nicht selten dem Alter zu. Alkoholmissbrauch oder -abhängigkeit werden kaum vermutet. Das Bundesamt für Gesundheit als Auftraggeberin der beiden Studien hält in der Nr. 93 von Spectra fest, dass die gesundheitlichen und ökonomischen Folgen des problematischen Alkoholkonsums im Alter aus Public-Health-Sicht ernst genommen werden müssen. Bleibt zu hoffen, dass die vorliegenden Erkenntnisse über den Alkoholkonsum der über 60-jährigen Bevölkerung in der Schweiz sowie die vorgeschlagenen Massnahmen zur Problemreduktion von der Politik wahrgenommen und vermehrt diskutiert werden. |

Abstract |
Les initiants de l’initiative fédérale pour la protection contre le tabagisme passif, et celles et ceux qui la soutenaient, ont été très déçus du résultat de la votation. Ce refus très net a d’autant plus étonné que, selon les premiers sondages, la majorité des citoyens interrogés soutenaient ce texte. Le résultat du 23 septembre est également regrettable parce qu’il pourrait avoir une influence négative sur les efforts des cantons pour améliorer la protection contre la fumée passive. La campagne de votation a été menée avec émotion, à coup de déclarations trompeuses, ce qui a sans doute désécurisé les citoyens qui venaient juste de s’adapter à la nouvelle loi fédérale. Celleci autorise les établissements fumeurs et les fumoirs avec service pour autant que les cantons ne prévoient pas de réglementation plus sévère. Cette initiative voulait protéger les tiers contre le tabagisme passif de façon encore plus complète qu’aujourd’hui. Très concrètement, elle voulait mieux protéger le personnel et les clients de la restauration et du secteur hôtelier. Addiction Suisse regrette que cette importante opportunité d’améliorer la protection de la santé ait été ratée, car de trop nombreux employés restent exposés de longues heures à la fumée nocive du tabac. Autre sujet d’intérêt : deux études récentes apportent de nouveaux éléments d’information sur la consommation d’alcool des personnes âgées. Alors que l’attention médiatique est dirigée vers la consommation d’alcool problématique des adolescents et des jeunes adultes, on n’accorde que peu d’attention aux comportements des personnes âgées. Ce sujet mérite pourtant plus d’attention. Il est difficile d’évaluer dans quelle mesure ces deux nouvelles études d’Addiction Suisse et de l’Institut de recherche sur les addictions et la santé publique (Institut für Sucht- und Gesundheitsforschung, ISGF) initieront un véritable débat public. Le fait est que les signes d’un abus d’alcool ou d’une dépendance ne sont souvent pas reconnus ou sont attribués à la prise de médicaments ou à une démence. Les proches attribuent souvent des dépressions ou de l’insomnie à un effet de l’âge et ne soupçonnent que rarement l’abus d’alcool ou la dépendance. L’Office fédéral de la santé publique (OFSP), qui a mandaté ces deux études, écrit dans son bulletin consacré à la prévention et la promotion de la santé (Spectra no. 93, page 9) que les conséquences sanitaires et économiques de la consommation problématique d’alcool par les personnes âgées doivent être prises au sérieux du point de vue de la santé publique. Il reste à espérer que le monde politique prendra conscience et débattra de façon accrue des connaissances acquises sur la consommation d’alcool de la population de plus de 60 ans en Suisse ainsi que des mesures proposées pour diminuer ce problème. |
