  Abstract La politique de réduction des risques a aujourd’hui plus de dix ans en France. Nous sommes d’ailleurs bien loin des premières actions «marginales», pour ne pas dire marginalisées, engagées à la fin des années 80 par quelques associations pionnières en la matière. Le paysage institutionnel et associatif s’est progressivement structuré, les méthodes se sont affinées et les outils multipliés. Sans vouloir crier victoire trop tôt, ce retard si spécifique qui frappait si durement les usagers de drogues injectables de l’hexagone semble en partie comblé et c’est avec surprise que nous nous retrouvons même, dans certains domaines, moteur d’une Europe étonnée de nos propres contradictions.

  Abstract With the limited exception of HIV/AIDS prevention, harm minimisation in the UK has focused on the harm that drug users may do to themselves. This article explains how harm minimisation within Hammersmith and Fulham (part of inner London ) has now begun to address wider issues. Hammersmith and Fulham has the highest estimate HIV+ rate of any local authority area in the UK, and approximately 4000 injecting drug users. Spending on HIV prevention and services for people with HIV/AIDS is the highest in the UK . Police estimate that more than half of all acquisitive crime in the area is committed to fund drug purchases. Public services in the health, criminal justice, education, housing and social fields have formed an unusually close partnership to address these problems. This is generating rapid changes in service provision, including moves away from prosecution of people in possession of any drug for their own use, and moves to attract many more users into treatment by greatly extending prescribing practice. The overall aims of harm minimisation in the area now include reduction of crime and reduction of the illicit drug market alongside minimisation of harm to drug users themselves.

  Abstract Die Ergebnisse einer Umfrage zum Gesundheitsverhalten von 8698 Schülerinnen und Schüler im Alter von 11 bis 15 Jahren zeigen: 11- bis 15jährige Schulkinder rauchen mehr; besonders bei den 15jährigen ist in der Zeitspanne von 1994 und 1998 ein starker Anstieg regelmässig Rauchender (mindestens wöchentlicher Konsum) festzustellen, und zwar sowohl bei Mädchen als auch bei Jungen. Rund 30% der täglich rauchenden Schulkinder haben schon erfolglos versucht, mit dem Rauchen aufzuhören; 5% hatten Erfolg. Rund die Hälfte bekundet keine Absichten, ihre Gewohnheit aufzugeben. Insgesamt trinken Schulkinder im Jahre 1998 nicht mehr Alkoholisches als vor vier Jahren. Werden die Daten nach Geschlechtern getrennt ausgewertet, so zeigt sich ein anderes Bild: Mädchen trinken in erhöhtem und Jungen eher in geringerem Ausmasse. Doch nach wie vor konsumieren männliche Heranwachsende mehr alkoholische Getränke als weibliche Jugendliche. Sowohl bei Jungen als auch Mädchen ist ein markanter Anstieg von Trunkenheitsereignissen festzustellen; sie trinken Alkohol nicht um der Neugier oder des Genusses willen, sondern vielmehr um sich “high” zu fühlen. Im Vergleich zu 1994 hat sich 1998 der Anteil der Cannabis-Erfahrenen um rund 10 Prozentpunkte erhöht. Offensichtlich wird der Konsum von Cannabis-Produkten bei Jugendlichen zunehmend banalisiert. Von besonderer Bedeutung ist der Sachverhalt, dass vor allem der Anteil jener Schüler und Schülerinnen gestiegen ist, die Cannabis mehrmals versucht haben. D.h. der Anstieg der Lebenszeitprävalenzen ist nicht in erster Linie einer Zunahme des Neugierkonsums zuzuschreiben, sondern indiziert vielmehr eine generelle Einbettung des Cannabiskonsums in jugendliche Konsumgewohnheiten. Rund 3 Prozent der 15jährigen haben Erfahrungen im Umgang mit halluzinogenen Pilzen, Stimulanzien (Amphetaminen) und illegal gehandelten Medikamenten. Für den Gebrauch von Ecstasy, Kokain und LSD liegen die Werte unter 2 Prozent. Folgerungen für die Prävention schliessen den Artikel ab. Les résultats d’une enquête sur le comportement de 8698 jeunes de 11-15 ans révèlent que cette tranche d’âge fume davantage. Entre 1994 et 1998, les fumeurs habituels augmentent chez les 15 ans, filles et garçons. 30% des étudiants fument tous les jours. Dans l’ensemble, les jeunes ne boivent pas plus d’alcool, mais la ventilation des statistiques montrent que les filles boivent plutôt plus et les garçons plutôt moins et les états d’ivresse augmentent. Le pourcentage d’individus ayant essayé le cannabis a progressé de 10% en 1998, la consommation se banalise et s’enracine dans les habitudes des jeunes. 3% des plus de 15 ans ont consommé des champignons hallucinogènes, des stimulants et des médicaments détournés. La consommation d’ecstasy, de cocaïne et de LSD reste inférieure à 2%. The results of a survey on the health-related behavior of 8,698 students aged 11 to 15 years show that they smoke more. Among 15-year-olds in particular, a significant increase in regular consumption (at least once a week) was found in girls and boys alike between 1994 and 1998. Some 30% of daily smokers have already tried unsuccessfully to quit smoking; 5% were successful. Around half has no intention to kick the habit. Overall, children of school age did not consume more alcohol in 1998 than four years prior. However, when findings are broken down according to sex, the picture is different: girls drink more and boys tend to drink less. Like before, however, male adolescents consume more alcoholic beverages than female youth. A marked increase in drunkenness is found among boys and girls alike. They do not drink alcohol because they are curious or because they enjoy it but because they want to feel high. Compared to 1994, the num her of individuals with cannabis experience jumped by 10 percentage points in 1998. Apparently, the consumption of cannabis products among youth is increasingly viewed as trivial. Of particular significance is the fact that the number of students who have tried cannabis several times has increased. In other words, the increase in lifetime prevalence is not the result of curiosity but much more an indication of an overall trend to include cannabis in the consumption habits of adolescents. Some 3 percent of 15-year-olds have consumed hallucinogenic mushrooms, stimulants (amphetamines) and illegally obtained medication.
