  Abstract Dans le champ du contrôle de l’usage des drogues illicites, la réduction des méfaits est souvent pensée comme une troisième voie en regard des logiques traditionnelles du prohibitionnisme et du thérapeutique orienté vers l’abstinence. L’analyse comparative de ces trois logiques amène les auteurs à établir les limites de la rupture induite par la réduction des méfaits. Par la cartographie du territoire que colonise la réduction des méfaits, s’aperçoit non pas la disqualification du prohibitionnisme et du thérapeutique, mais l’extension des dispositifs de contrôle. Après avoir conduit cette analyse et commenté certains effets du concubinage des trois logiques, les auteurs présentent plusieurs thèses sociologiques permettant de signifier les succès politiques de la réduction des méfaits. Quelques interprétations de ces succès sont proposées. Within the context of illicit drug control, harm reduction is often regarded as the third avenue beside the traditional responses that represented the prohibitionnist model and the therapeutic model oriented towards abstinence. Through a comparative analysis of the three approaches, the authors challenge the notion that harm reduction represents a rupture regarding the traditional perspectives on drug use. On the contrary, an examination of how harm reduction strategies are implemented in the field of drug regulation illustrates instead a widening of the apparatus of control. Following a presentation of the cohabitation between these three approaches, the authors then outline a sociological framework for understanding the political appeal of harm reduction strategies. Several reasons for the success of these strategies are then proposed. En el campo del control del uso de drogas ilícitas, la reducción de perjuicios es pensada a menudo como una tercer vía en mira de las logísticas tradicionales del prohibicionismo y de lo terapeútico orientado hacia la abstinencia. El análisis comparativo de éstas tres lógicas conlleva a los autores a establecer los límites de la ruptura inducida por la reducción de los perjuicios. Por la cartografía del territorio que coloniza la reducción de perjuicios se entrevee no la discalificación del prohibicionismo y de la terapéutica, pero la extensión de los dispositivos de control. Después de haber conducido éste análisis y comentado ciertos efectos del concubinage de estas tres lógicas, los autores presentan varias tésis sociológicas que permiten significar el éxito político de la reducción de perjuicios. Algunas interpretaciones de éstos éxitos nos son propuestas

This year reviews on the addictions emphasizes five aspects, on a bio-psycho-social perspective: (1) The relationship between methadone and cardiotoxicity. (2) The introduction of Eye Movement Desensibilization and Reprocessing (EMDR). (3) The apparition of a possible specific pharmacotherapy for excessive gambling. (4) A better knowledge of the relationship between cannabis and psychoses. (5) Resistance to treatment in the doctor-patient relationship.

  Abstract Using a sample of 614 youths, this study examined differences and similarities between adolescents at risk (age range 11-20 years), and their integration in school or vocational training. We found that mood states (depression, anxiety), secure self (self-esteem and self-efficacy), relationship with parents, delinquency and substance use (alcohol, tobacco and cannabis) were variables which largely influenced social integration (school, work) for youths with poor sociodemographic backgrounds. An important variable we introduced here was sociodemographic background, which investigated the social situation of a youth. We distinguished two different groups (good vs. poor sociodemographic background). There were important differences between both groups. Youths with a poor sociodemographic background scored higher in suicidality and health complaints. The most striking differences were in substance use and delinquency. The structural equation model showed that poor mood was related to substance use at the start. Important paths were found in mood states over time, as for substance use, but interactions between both variables were small. We ran different models (poor vs. good social background, Swiss vs. migrant youths), and concluded that secure self and good family relations protected against poor mood and substance use and both variables indirectly influenced integration 2-3 years later.

  Abstract Recent major epidemiological surveys in general population samples throughout the world have demonstrated that substance use disorders are among the most frequent forms of mental disorders in the community, and are also frequently associated with other forms of mental disorders. This paper briefly reviews the concept of comorbidity and summarizes more recent data concerning the frequency of comorbidity of substance use disorders. The review is limited to studies in the general population using standardized diagnostic interviews. Specific emphasis is laid upon the most recent data from the National Comorbidity Survey, that specifically addressed comorbidity issues in detail. The clear majority of subjects with a definite substance use disorder according to the strict DSM-III-R definition has or has had at least one other comorbid mental disorder. Comorbidity rates between specific substance use disorders and other mental disorders will be compared and discussed in light of several other international epidemiological studies. Furthermore time sequences of substance use disorders and comorbid disorders are presented. Potential pathogenetic and clinical implications are addressed.
