Abstract Throughout the year, bars, nightclubs, discotheques and other recreational venues provide young Europeans with opportunities to socialise and dance, often offering entertainment such as concerts and dance parties. In addition, large music festivals that are organised mostly during the summer months attract thousands of visitors. Surveys show that in many recreational venues the use of drugs is more prevalent than in the general population (EMCDDA, 2006).

Abstract Depuis des décennies, nous menons une guerre acharnée contre le dopage. Celle-ci n’est pas sans rappeler la « guerre à la drogue » où la stratégie poursuivie ne permet pas d’atteindre le but déclaré (l’éradication) et provoque des effets collatéraux néfastes. Des spécialistes tirent la sonnette d’alarme et rejoignent les revendications des professionnels des addictions, qui militent pour l’abandon de la tolérance zéro et l’adoption d’approches pragmatiques dans l’intérêt de la santé publique.

Abstract In letzter Zeit scheint das Bestreben gesunder Menschen, ihre Leistungsfähigkeit im Berufs- und Privatleben mittels Eingreifen in die Funktionsweise des Körpers zu steigern, zuzunehmen – zumindest, wenn man den Medienberichten Glauben schenkt. Im körperlichen Bereich hat sich das Optimierungsstreben mit der allgegenwärtigen Schönheits- und Leistungsfixierung und den entsprechenden Angeboten an Life-Style-Medikamenten (z. Bsp Anti-Aging Produkte) Schönheitschirurgie und Sportmedizin zumindest in gewissen Kreisen bereits durchgesetzt. Im Sport haben die gesundheitlichen Risiken des Dopings, vor allem aber die weit verbreitete Sichtweise, dass sich Wettkampfsportler und –sportlerinnen auf unfaire Weise Vorteile verschaffen, eine weitreichende ethische und philosophische Debatte sowie die Etablierung von Regeln zum Gebrauch von leistungssteigernden Substanzen ausgelöst. Die öffentliche Diskussion um Leistungssteigerung im Berufs- und Privatleben hingegen steht noch am Anfang. Mit dem Grundlagenpapier will Sucht Schweiz eine Orientierungshilfe bieten, indem der gegenwärtige Diskussionsstand zu Doping im Alltag aufgezeigt und erste Schlussfolgerungen gezogen werden. Dabei wird die aktuelle Debatte mit Bezug auf die “Sucht-Trias”, das heisst betreffend Substanzen, Individuum sowie Gesellschaft, beleuchtet. Es scheint uns unabdingbar, nebst den individuellen auch die gesellschaftlichen und ethischen Aspekte in die Bewertung bzw. in die Risikoabwägung einzubeziehen.

Abstract A en croire les médias, on assiste à une augmentation de la tendance à recourir aux artifices les plus divers pour augmenter sa performance au travail et dans la vie privée. Même des personnes en parfaite santé n’hésitent pas à intervenir dans le fonctionnement de leur corps. Une pratique qui n’a rien de nouveau. Dans le domaine du physique ce souci d’optimisation, nourri par l’obsession contemporaine de la beauté et de la performance, se traduit déjà, du moins dans certains milieux, par la chirurgie esthétique, la médecine sportive et les médicaments “lifestyle” (par exemple les produits anti-âge). En matière de sport, le dopage a déjà provoqué un vaste débat éthique et philosophique, non seulement en raison des risques qu’il comporte pour la santé, mais surtout du manque de fair-play dont les sportifs de compétition font preuve, de l’avis général, en se procurant ainsi des avantages injustifiés sur leurs adversaires. Cette controverse a débouché sur l’établissement de règles concernant l’utilisation des produits dopants. En revanche, la discussion sur l’emploi de ces derniers dans le cadre de la vie privée et professionnelle ne fait que commencer. Avec ce présent document, Addiction Suisse souhaite présenter les questionnements actuels sur le sujet et proposer quelques premières conclusions sur le phénomène. Elle entend donner au débat l’éclairage spécifique de l’approche bio-psycho-sociale, qui se réfère à la fois au produit, à l’individu et à la société dans laquelle il évolue. Il nous semble en effet indispensable d’inclure dans le débat, comme dans l’estimation des risques, les aspects sociétaux et éthiques en plus de ceux qui sont propres à l’individu.

Abstract As of 10 July 2012, five cases of anthrax have been reported among injecting drug users (IDUs) from Germany (n=3), Denmark (n=1) and France (n=1). Reported dates of onset in all five cases range between early June 2012 and 11 July 2012. One German case and the case from Denmark have died. The first two cases from Germany are likely linked through exposure to heroin contaminated by a most likely identical Bacillus anthracis strain (based on molecular typing results). For the third German case there is also some laboratory evidence that the strain could be identical with the outbreak strain, but due to the limited amount of DNA, further typing is not possible. The link of the remaining two cases, though probable, needs to be confirmed through molecular typing. The geographical distribution of the potentially contaminated heroin is unknown at this time, but it is possible it has the same source as the contaminated heroin incriminated in the 2009/2010 outbreak in Scotland (with cases also reported from Germany and England). The risk of exposure for heroin users in European Union (EU) countries is presumably still present and therefore it is not excluded that additional cases among IDUs will be identified in the near future. Information could be disseminated to health care workers, drug treatment and harm reduction centres describing the symptoms of anthrax infection to ensure early treatment, and urging the provision of appropriately-dosed opiate substitution treatment to prevent further anthrax cases.
