Abstract |
Alters- und Pflegeheime sind zunehmend von Diversität betroffen. Auf der einen Seite steht die Komplexität der alten Menschen, die sich in den Institutionen durch Hochaltrigkeit, Multimorbidität, Demenz und andere gerontopsychiatrische Auffälligkeiten ihrer Bewohnerschaft zeigt. Auf der anderen Seite ist auch das Pflege- und Betreuungspersonal von grosser Vielfalt geprägt. Die Wegleitung zeigt anhand von Untersuchungsergebnissen die Risiken für vulnerable Bewohnerinnen und Bewohner von Altersinstitutionen und bietet praxisnahe Handlungsmöglichkeiten für den Umgang mit Diversität. Die Schulungs-DVD, mit Spielszenen und Kommentaren von Expertinnen und Experten, eignet sich für den Einsatz in Aus- und Weiterbildung von Pflege- und Betreuungspersonal sowie für Sensibilisierungsveranstaltungen. |

Abstract |
Mehrere Studien zeigen, dass eine Unterversorgung der ausländischen PatientInnen im Vergleich zu den einheimischen PatientInnen im ambulanten psychiatrischen Bereich besteht. MigrantInnen wenden sich mit ihren Beschwerden meist an somatische ÄrztInnen und werden oft erst sehr spät an psychiatrische Fachleute weiterverwiesen, so dass dann oft bereits eine Chronifizierung ihrer psychischen Erkrankung vorliegt. Hohe Kosten für das Gesundheitssystem und geringere Rehabilitationschancen sind die Folgen. Zur Förderung der Chancengleichheit betreffend Zugang zum Gesundheitssystemist es notwendig, in der Schweiz vermehrt ambulante psychotherapeutische und psychiatrische Angebote aufzubauen, welche auf den Aspekt der Migration spezialisiert sind. Die Sprechstunde für MigrantInnen der Universitären Psychiatrischen Dienste Bern (UPD) bietet ein niederschwelliges, ambulantes, multimodales Behandlungskonzept für psychisch kranke MigrantInnen, und insbesondere auch für Asylsuchende und traumatisierte MigrantInnen an. Durch den Einsatz verschiedener Module kann die Behandlung der PatientInnen je nach individuellen Bedürfnissen und Möglichkeiten unterschiedlich ansetzen. Durch den Einbezug von geschulten ÜbersetzerInnen wird der sprachliche Zugang erleichtert. Insbesondere Gruppenangebote stellen eine ressourcen- und kostensparende Möglichkeit dar, vielen PatientInnen Unterstützung anzubieten. Das vorliegende Handbuch gibt Einblick in ein bew ährtes Behandlungskonzeptund regt zur Nachahmung an. |

Abstract |
Plusieurs études montrent que l’offre thérapeutique destinée aux patientsé trangers dans le domaine psychiatrique ambulatoire est insuffisante en comparaison de l’ offre à la disposition des patients suisses. La plupart du temps, les migrants s’ adressent aux médecins somatiques et ne sont envoyés que bien plus tard chez les psychiatres, lorsque les troubles psychiques sont déjà devenus chroniques. Il en résulte des coûts élevés pour le système de santé et des possibilités de traitement plus limitées. Afin de permettre aux migrants souffrants de troubles psychiques l’ égalité des chances d’ accéder au système de santé, il est nécessaire de développer un plus grandnombre d’ offres psychothérapeutiques et psychiatriques spécifiques à la migration en Suisse. La Consultation pour migrants des Services psychiatriques universitaires deBerne (SPU) propose un concept de traitement ambulatoire, multimodal et à bas seuil destiné aux migrants atteints d’ un trouble psychique, en particulier aux requérants d’ asile et aux migrants ayant subi un traumatisme. Différents modules permettent detraiter les patients en misant sur plusieurs approches. Le recours à des traducteurs qualifiés permet la compréhension mutuelle. Les offres de thérapie de groupe, en particulier, sont un moyen d’ aider de nombreux patients tout en économisant des ressources et des coûts. Le présent manuel donne un aperçu d’ un concept de traitement éprouvé et appelleà faire école. |

Le Fonds de recherche Société et culture Québec (FQRSC) vient de rendre disponible la thèse de Cathy Savard de l’Université Laval portant sur le jeu chez les adolescents et plus particulièrement sur les adolescents à risque de développer un problème relatif aux jeux de hasard et d’argent. Cette thèse a pour but “de proposer un nouveau modèle explicatif sur le développement et le maintien des problèmes de jeu à l’adolescence, et d’étudier les habitudes de jeu des adolescents en centre jeunesse (CJ), un sous-groupe réputé pour présenter plusieurs facteurs associés au jeu.” (Savard, 2012 ; p. 74) |

Abstract |
Dysfunctional international organizations are anomalies in world politics. Neither rationalist nor constructivist attempts to explain their existence, persistence and growth have provided viable results, so far. However, beyond enhancing efficiency and effectiveness of international cooperation, less obvious functions of international organizations exist. Drawing on a primarily realist perspective, this study argues that states can use international organizations to obscure unwillingness to cooperate or solving common problems and diffuse responsibility or shift blame when commonly agreed upon solution do not work out as expected. In such situations, dysfunctions of international organizations might even be welcomed by states, because they provide obvious reasons for failure and make blame-shifting strategies more credible. This argument is explored in a case study of the United Nations Office on Drugs and Crime (UNODC) – a dysfunctional yet growing international organization. The case study argues that states have only a limited interest in cooperating on drug control matters at the international level. Co-existence rather than cooperation is the major aim states pursue. Because states do not want to leave the achieved state of co-existence, they are not interested in a strong and functioning international organization in the field. The growth of the organization can be almost exclusively attributed to the technical assistance projects of the UNODC. However, a closer look at a recent high-profile assistance project shows that even in the operative realm states are not interested in the success of the organizations. Much more, they use the UNODCs technical capabilities to obscure their unwillingness to cooperate substantially on drug control matters when their interests are only marginally affected. The study concludes that in the case of the UNODC blame-shifting by states is one of the main reasons for the UNODC’s budget growth despite its dysfunctions. Although it remains unclear to what extent this insight can be generalized to all dysfunctional international organizations, the mechanisms identified seem to provide viable explanations for other dysfunctional organizations operating under similar circumstances. |
