Abstract |
This paper documents patterns and sequence of initiation of club drug use in a sample of 450 gay and bisexual men in New York City. Quantitative and qualitative baseline data from a yearlong longitudinal investigation conducted between 2001 and 2005 were analyzed. The study focused on the use of five club drugs – cocaine, GHB, ketamine, ecstasy, and methamphetamine – using self-reported indications of use for a period of 4 months prior to assessment. Patterns of club drug use among gay and bisexual demonstrated that poly-club-drug use is common, and that patterns of use can be differentiated along the lines of age, race/ethnicity, and sexual orientation, with those who are older, Black, and bisexual, reporting less club drug use. The majority of the men initiated use of the five club drugs as follows: (a) cocaine, (b) ecstasy, (c) ketamine, (d) methamphetamine, and (e) GHB. Variations in patterns were related to both age and level of poly-club-drug use. The sequencing and/or patterns of club drug use may be better explained by socialization processes in the gay community than by Gateway Theory, which has been traditionally used to explain patterns of drug use in the population. Future research should more closely examine the synergy of drug use combinations with an emphasis on measuring the extent to which the drugs are taken in synchronicity. |

- Le médecin de premier recours a un rôle essentiel dans le dépistage du jeu excessif en sa qualité de dépositaire du secret médical. – Plus du tiers des demandes d’aide liées au jeu excessif sont associées à des idées suicidaires lors de la première consultation. – Le traitement est multimodal et multifocal et il doit combiner notamment la clarification des motivations à réduire ou non l’activité de jeu, le soutien aux proches, des restrictions d’accès au jeu et/ou à l’argent, l’intégration de l’espoir de gain structurellement négatif, et l’abord des comorbidités anxiodépressives. – Il existe en Suisse un réseau de centres et antennes de prises en charge spécialisée accessibles par le biais d’un numéro vert et des sites: www.sosjeu.ch / www.sosspielsucht.ch / www.giocoresponsabile.com. |

Abstract |
- Dem Hausarzt kommt als Hüter des Arztgeheimnisses bei der Früherkennung exzessiven Spielverhaltens eine wichtige Rolle zu. – Mehr als ein Drittel derjenigen, die erstmals einen Arzt wegen exzessiven Spielverhaltens aufsuchen, hat Suizidgedanken. – Die Behandlung beinhaltet verschiedene Modalitäten und Angriffspunkte; dazu gehören insbesondere die Abklärung der Motivation, weniger zu spielen, der Unterstützung durch Angehörige oder Freunde und der Möglichkeit, den Zugang zu Spielen und/oder finanziellen Mitteln einzuschränken. Auch die strukturell negative Gewinnerwartung und die ängstlich-depressiven Begleiterkrankungen müssen einbezogen und angegangen werden. – In der Schweiz besteht ein spezialisiertes Netz von Kontaktstellen und Behandlungszentren, die über eine Gratisnummer oder im Internet unter www.sos-spielsucht.ch / www.sos-jeu.ch / www.giocoresponsabile.com erreichbar sind. |

Abstract |
We conducted a pilot study (N = 22) comparing the efficacy of desipramine and amantadine for treatment of cocaine dependence in methadone maintenance clients. The study which lasted 12 weeks, was double-blind, randomly assigned, and placebo-controlled. Subjects met DSM-III-R criteria for active cocaine dependence. All three groups’ cocaine use, craving, and depressive symptoms declined significantly, but intergroup differences were not significant. Clients receiving desipramine were significantly more likely to remain in treatment and to be cocaine free at study completion. The results emphasize the importance of delivering comprehensive services to the cocaine user in methadone treatment. Further evaluations of these two medications as adjuncts in the treatment of cocaine dependence are needed. |
