  Abstract L’évolution de l’action collective en matière de traitement et de réinsertion dans le champ des addictions s’inscrit dans un processus de renforcement et d’élargissement de l’offre – résidentielle et ambulatoire – de prise en charge des personnes dépendantes. Une tendance à l’affaiblissement de la place du résidentiel est identifiée en Suisse depuis la fin des années 1990. Cependant, face à la prévalence de certaines situations sociales et psychopathologiques lourdes et à l’évolution des profils des usagers, cette relativisation de la place du résidentiel semble avoir plus récemment trouvé ses limites. Les différents acteurs des dispositifs de traitement reconnaissent que l’addiction est un phénomène qui résulte de l’interaction de multiples facteurs. La recherche de solutions implique la concertation de plusieurs domaines complémentaires: le social, la santé, l’éducation et la sécurité. Dans cette perspective multifactorielle et interdisciplinaire, la sortie de la dépendance ne concerne pas seulement l’affranchissement de pratiques de consommation, mais aussi l’éloignement de situations de vie problématiques. Une «convergence adaptative» se dessine ainsi entre les approches médicale et sociale et les acteurs ambulatoire et résidentiel posant la question de leur articulation. Dès lors, comment intégrer les héritages des référentiels et de leurs dispositifs d’action respectifs sans diluer les compétences acquises? Et quelles pistes de réflexion peuvent inspirer des reconfigurations possibles de cette politique publique? Ce sont les questionnements auxquels l’Observatoire de la Ville et du Développement durable de l’Institut de Géographie de l’Université de Lausanne contribue à répondre dans une étude évaluative sur les institutions romandes publiée récemment1. Le présent article reprend et met en perspective les principaux enjeux que celle-ci a pu mettre en évidence. L’evoluzione dell’azione collettiva in materia di trattamento e di reinserimento nel campo delle dipendenze si iscrive in un processo di rafforzamento e di ampliamento dell’offerta – residenziale e ambulatoriale – di presa a carico delle persone dipendenti. Una tendenza all’indebolimento del ruolo del residenziale si evidenzia in Svizzera dalla fine degli anni 1990. Tuttavia, di fronte alla prevalenza di certe situazioni sociali e psicopatologiche pesanti e all’evoluzione dei profili degli utenti, questa relativizzazione del ruolo del residenziale sembra avere recentemente trovato i suoi limiti. I diversi attori dei dispositivi di trattamento riconoscono che la dipendenza è un fenomeno che risulta dall’interazione di molteplici fattori. La ricerca di soluzioni implica la concertazione di più campi complementari: il sociale, la sanità, l’educazione, la sicurezza. In questa prospettiva multifattoriale e interdisciplinare, l’uscita dalla dipendenza non riguarda solo l’affrancamento dalle pratiche di consumo, ma anche l’allontanamento dalle situazioni di vita problematiche. Una “convergenza di adattamento” si disegna fra gli approcci medico e sociale e gli attori dei settori ambulatoriale e residenziale, ponendo la questione della loro articolazione. Quindi, come integrare le diverse prospettive e i rispettivi dispositivi d’azione senza diluire le competenze acquisite? E quali piste di riflessione possono nascere dalle riconfigurazioni possibili di questa politica pubblica? Sono le domande alle quali l’Osservatorio della Città e dello Sviluppo sostenibile dell’Istituto di Geografia dell’università di Losanna contribuisce a rispondere in uno studio di valutazione sulle istituzioni romande pubblicato recentemente.Il presente articolo riprende e mette in prospettiva le principali sfide che lo studio ha potuto mettere in evidenza.

  Abstract Die Schweiz bietet mit einer fortschrittlichen Drogenpolitik und dem gesetzlich verankerten Ansatz der Schadensminderung gute Voraussetzungen, um als Drogenkonsument oder -konsumentin gesund zu bleiben oder es wieder zu werden. Gross angelegte Aufklärungskampagnen über das HIV-Ansteckungsrisiko und Aids haben in den letzen zwei Jahrzehnten enorme Erfolge erzielt. Im Bereich der Hepatitis C sieht die Bilanz ernüchternder aus. Über 50 Prozent der intravenös Drogen Konsumierenden (aktiv oder ehemals) sind mit dem Hepatitis-C-Virus (HCV) infiziert. Obschon viele von ihnen in medizinischer Betreuung sind, werden sie nur selten auf eine mögliche Infektion getestet, und wenn ein positives Resultat vorliegt, wird eine Therapie noch immer sehr zurückhaltend angeboten. Eine unbehandelte Hepatitis C kann jedoch zu Leberschädigungen bis hin zu Leberversagen und damit zum Tod führen. Das Projekt der Testings in den Kontakt- und Anlaufstellen (K&A) der Stadt Zürich soll die Konsumierenden selber, aber auch die in der Suchtversorgung tätigen Institutionen für das Problem der Hepatits-C-Infektionen sensibilisieren und über Behandlungsmöglichkeiten aufklären. Die Auswertung des Pilotprojekts zeigt, dass das Bedürfnis nach Information gross ist – die Weichen für entsprechende Folgeprojekte sind somit gestellt.
