Abstract The paper examines the influence of the real price of beer on violence-related injuries across the economic regions in England and Wales. The data are monthly frequency of violent-injury collected from a stratified sample of 58 National Health Service Emergency Departments 1995-2000. An econometric model based on economic, socio-demographic and environmental factors was estimated using panel techniques. It is shown that the rate of violence-related injury is negatively related to the real price of beer, as well as economic, sporting and socio-demographic factors. The principal conclusion of the paper is that the regional distribution of the incidence of violent injury is related to the regional distribution of the price of beer. The major policy conclusion is that increased alcohol prices would result in substantially fewer violent injuries and reduced demand on trauma services.

Abstract Auf Basis internationaler Standards orientiert sich die Gesundheitsversorgung Gefangener auch in Deutschland am „Äquivalenzprinzip“ (Gleichwertigkeit der medizinischen Versorgung innerhalb der Gefängnisse mit der im jeweiligen Land vorgehaltenen medizinischen Versorgung). Vor diesem Hintergrund werden anhand ausgewählter Gesundheitsprobleme nicht nur vereinzelte Angleichungs- und Umsetzungsprobleme in der Gesundheitsversorgung Gefangener verdeutlicht, sondern auch strukturelle Probleme benannt, die aus dem von der GKV entkoppelten Parallelsystem der gefängniseigenen „Gesundheitsfürsorge“ resultieren. Die vorliegende Übersichtsarbeit dient als Grundlage für Vorschläge zum Umdenken und zur Reorganisation der intramuralen Gesundheitsdienste. On the basis of international standards, health care in German prisons has been oriented along the principle of equivalence (equity of health care delivery for inmates compared with the health care delivery in the community). Against this background, selected health problems demonstrate not only isolated problems in adaptation and implementation of health care delivery, but also structural problems resulting from the parallel system of prison health care, which is separated from the general German health care insurance system. This review serves as a basis to present suggestions to rethink and reorganize prison-based health care services.

Abstract Einer Einführung zum Stande der Forschung zum moderaten Alkoholkonsum folgen eine Gesamtübersicht und die Kategorisierung von Behandlungsangeboten in der Schweiz zum Kontrollierten Trinken (KT) sowie von Ansätzen zur Konsumsteuerung ambulant und stationär. Erste Ergebnisse einer Internetumfrage bei n = 34 KT-Therapeutinnen und Therapeuten 2009 erlauben einen Vergleich zwischen der Kurspraxis und den Kurskonzepten alcochoix+ und dem KT-Modell nach Körkel. Entgegen den konzeptuellen Programmvorgaben bejahen die KT-Therapeuten die Anwendung auch bei abhängigen Klienten und konstatieren Individualisierungsbedarf. Controlled drinking programs in Switzerland – a look into the black box A short introduction on alcohol and moderation is followed by an overview of controlled drinking programs (cd) in Switzerland as well as approaches to limit consumption (,wet places‘). The results of an internet survey conducted in 2009 among n = 34 treatment staff who run cd-programs allow for a comparison between treatment practice and program concepts in the French (adapted version of the Canadian program alcochoix+) and the German speaking part (German cd program according to Körkel) of Switzerland. Contrary to officially declared program limitations cd therapists agree that cd treatment should also be offered to dependent alcohol users and require a more individualized assessment. Recruiting clients for cd programs and competing claims of abstinence oriented treatment providers are reported as key issues – even though frequent client flows from cd to abstinence programs can be observed.

Abstract Jedes Kind hat Anrecht auf eine möglichst gesunde Entwicklung und auf eine gerechte Chance, später als Erwachsener einen angemessenen Platz in der Gesellschaft einzunehmen. Die Gesellschaft ihrerseits hat ein Interesse an gesunden Kindern und Jugendlichen, damit diese als Erwachsene ihre produktive Rolle auch tatsächlich wahrnehmen können und nicht der Gemeinschaft zur Last fallen. Die Präventionsforschung zeigt allerdings, dass die Prävention und Gesundheitsförderung nicht bei allen Themen gleich wirksam sein können und die Erwartungen somit auch nicht zu hoch geschraubt werden sollen. Auch kann die Prävention wichtige Startbedingungen ins Leben kaum beeinflussen und wird deshalb höchstens einen kleinen Beitrag zur Verbesserung der Chancengleichheit leisten können. Prévention: plus facile à dire qu’à faire Chaque enfant a droit à un développement aussi sain que possible et à une chance équitable d’occuper plus tard, à l’âge adulte, une place adéquate dans la société. La société de son côté a intérêt à compter des enfants et des jeunes en bonne santé pour que, devenus adultes, ils puissent véritablement jouer leur rôle productif, et ne tombent pas à la charge de la communauté. La recherche sur la prévention montre toutefois que la prévention et la promotion de la santé ne peuvent revêtir la même efficacité sur tous les thèmes, et qu’il ne faut pas créer des attentes trop ambitieuses. La prévention ne peut du reste guère influencer les conditions existentielles initiales importantes, et ne pourra donc fournir tout au plus qu’une faible contribution à l’amélioration de l’égalité des chances.

Title Results of a type 2 translational research trial to prevent adolescent drug use and delinquency : a test of Communities That Care Type Journal Article

Abstract Objective To test whether the Communities That Care (CTC) prevention system reduces adolescent alcohol, tobacco, and other drug use and delinquent behavior communitywide. Design The Community Youth Development Study is the first randomized trial of CTC. Setting In 2003, 24 small towns in 7 states, matched within state, were randomly assigned to control or CTC conditions. Participants A panel of 4407 fifth-grade students was surveyed annually through eighth grade. Intervention A coalition of community stakeholders received training and technical assistance to install the CTC prevention system. They used epidemiological data to identify elevated risk factors and depressed protective factors in the community, and chose and implemented tested programs to address their community’s specific profile from a menu of effective programs for families, schools, and youths aged 10 to 14 years. Main Outcome Measures Incidence and prevalence of alcohol, tobacco, and other drug use and delinquent behavior by spring of grade 8. Results The incidences of alcohol, cigarette and smokeless tobacco initiation, and delinquent behavior were significantly lower in CTC than in control communities for students in grades 5 through 8. In grade 8, the prevalences of alcohol and smokeless tobacco use in the last 30 days, binge drinking in the last 2 weeks, and the number of different delinquent behaviors committed in the last year were significantly lower for students in CTC communities. Conclusion Using the CTC system to reduce health-risking behaviors in adolescents can significantly reduce these behaviors communitywide.

Abstract Introduction – Des travaux récents en population adulte suggèrent que la position sociale détermine différemment les usages de produits psychoactifs licites et illicites des hommes et des femmes, aussi bien parmi les actifs occupés que les jeunes adultes, étudiants, chômeurs ou exerçant un emploi. Ce travail prolonge ces analyses auprès des jeunes de 17 ans en examinant plus précisément leur situation scolaire. Matériel et méthodes – Une enquête nationale représentative des jeunes de 17 ans de nationalité française menée régulièrement entre 2000 et 2005 permet de mesurer les évolutions des usages des principaux produits psychoactifs suivant le sexe, et de mettre en évidence leurs déterminants sociaux. Résultats – Le tabagisme quotidien a baissé, la consommation régulière d’alcool est restée stable, mais les ivresses alcooliques ont progressé, ainsi que la plupart des expérimentations de produits illicites, qui présentent une tendance à la féminisation, parfois marquée comme celles du crack et des amphétamines. Tous ces usages apparaissent plus unisexes au sein des jeunes scolarisés en collège et lycée qu’au sein des jeunes en apprentissage ou des déscolarisés, qui s’avèrent plus masculins. Conclusion – Les usages sont modulés par la situation scolaire et le milieu social des parents, mais seule la situation scolaire influence leur plus ou moins forte masculinité, plus marquée au sein des filières d’enseignement professionnelle ou parmi les jeunes sortis du système scolaire.

Abstract Objectifs – Évaluer l’influence du genre sur les usages de substances psychoactives parmi les adultes en France, suivant le niveau d’éducation, le statut face à l’emploi, la profession et catégorie sociale. Méthode – Le Baromètre santé 2005 est une enquête téléphonique nationale représentative des 12-75 ans suivant un sondage à deux degrés (ménages, individus) incluant les téléphones portables. Cette étude est centrée sur les 24 674 individus âgés de 18 à 64 ans. Les données ont été analysées à l’aide de régressions logistiques ajustées sur l’âge et trois indicateurs de situation sociale. Résultats – Les niveaux d’usage sont plus élevés parmi les hommes. Pour les deux sexes, une surconsommation de tabac est liée au chômage. La consommation d’alcool et l’ivresse alcoolique sont plus communes parmi les hommes sans emploi, mais pas parmi les femmes, chez qui la consommation d’alcool, l’ivresse et le cannabis sont plus fréquents chez les cadres que chez les ouvriers, ce qui n’est pas le cas pour les hommes. Des résultats similaires sont observés pour le niveau de diplôme, associé à des consommations plus élevées d’alcool, des ivresses plus fréquentes et un usage de cannabis plus répandu parmi les femmes, mais moins élevé parmi les hommes. Par conséquent, pour ces usages, les écarts entre les hommes et les femmes diminuent avec l’élévation du milieu social, quel que soit l’indicateur retenu. Conclusion – Les hommes sont généralement plus consommateurs de drogues que les femmes, mais l’ampleur de la différence varie suivant le milieu social : lorsqu’ils occupent des positions plus favorables, les hommes tendent à adopter des comportements d’usages plus raisonnables, tandis que les femmes ont tendance à les « masculiniser ». Gender issue in the analysis of addictive behaviours observed in the Baromètre santé study, France, 2005 Objectives – Assess the influence of gender on the use of psychoactive substances among adults in France depending on the level of education, and employment, occupational, and social status. Method – The present study (Baromètre santé) is a national representative telephone survey conducted in 2005 (two stages sample, household and individuals, mobile phones included) among 12-75-years-old. It focuses on 24,674 subjects aged 18-64 years. The data were analyzed using logistic regression adjusted on age and three social status indicators. Results – Psychoactive substances use is more prevalent in men. For both sexes, over-consumption of tobacco is related to unemployment. alcohol consumption and drunkenness are more common among unemployed men but not among women, in whom along with cannabis, they are more frequent among senior executives than among unskilled workers, which is not the case for men. Similar results are observed about the level of education, which is linked to a greater level of alcohol and cannabis use among women, but lower among men. Therefore, the gap between genders narrows as the social status increases, whatever the indicator used. Conclusion – Men generally consume more drugs than women, but the extent of differences varies depending on the social environment: when they have more favorable occupations, men tend to adopt more reasonable behaviours, whereas women’s behaviours tend to compare with men’s behaviours.
