  Abstract Die dialektisch-behaviorale Therapie (DBT) wurde ursprünglich für suizidale Borderline-Patientinnen entwickelt und wird mittlerweile in Form DBT-basierter Psychotherapien auch bei anderen Patientengruppen erfolgreich eingesetzt. In diesem Artikel werden die bisherigen Studien zur DBT-Behandlung von Borderline-Patienten mit komorbider Drogenabhängigkeit diskutiert und eine eigene Anwendung der DBT bei Alkoholpatienten mit komorbider Borderline-Persönlichkeitsstörung in der stationären Entwöhnungsbehandlung beschrieben. Da bei Patienten mit Alkoholabhängigkeit und komorbider Borderline-Persönlichkeitsstörung mit einer schwereren klinischen Problematik und weniger befriedigenden Therapieverläufen zu rechnen ist, ist zu hoffen, dass durch diese neuen Ansätze die Therapieergebnisse bei dieser Patientengruppe verbessert werden können. Dialectical behavior therapy (DBT) was originally developed for suicidal female patients with borderline personality disorder (BPD). Meanwhile, DBT-based approaches to psychotherapy have also been successfully applied in other clinical groups. Previous studies of DBT in patients suffering from BPD and comorbid drug addiction are discussed, and an approach to DBT that has been devised by the authors for use in the treatment of alcoholics with comorbid BPD is described. As these patients have more severe clinical problems and less satisfactory treatment responses than do alcoholics without comorbid BPD, we must hope that this new approach will improve clinical outcomes in these severely ill patients.

  Abstract Research is an increasing priority for workers throughout the mental health sector, and therapeutic communities are no exception. Those working in TCs increasingly have to justify the success and efficiency of their methods to outside bodies, and the prime means of doing so is through research. This volume collects a wide range of papers by experienced contributors discussing all aspects of TC research. They consider questions of which methods are most appropriate in the unique environment of TCs, how research studies affect the TC environment, as well as practical and ethical questions. The book also includes accounts of several research studies undertaken at, among other places, the Cassel Hospital. Taken as a whole, the book will be an invaluable resource for anyone involved in researching therapeutic communities – whether undertaking a study, attempting to anticipate its clinical impact, or seeking to understand the kinds of results which TC research produces.

  Abstract Fragestellung: Welche neurobiologischen Erkenntnisse zu Ursachen und Folgen übermäßigen Alkoholkonsums bei Kindern und Jugendlichen sind gegenwärtig bekannt? Methodik: Überblick über einschlägige epidemiologische und experimentelle Studien. Ergebnisse: Eine Reihe neurobiologischer Faktoren beeinflusst das Trinkverhalten Jugendlicher und das Risiko für spätere Alkoholabhängigkeit. Sie sind größtenteils genetisch beeinflusst, entfalten ihr Risikopotential jedoch erst durch wechselseitige Interaktion mit Umwelteinflüssen. Bereits im Jugendalter können Alkoholfolgeschäden am Gehirn und seiner Funktion auftreten. Schlussfolgerungen: Das Vermeiden übermäßigen Alkoholkonsums im Jugendalter begünstigt den noch im Ablauf befindlichen Prozess der Gehirnreifung und verringert die Gefahr späterer Alkoholsucht. Aim: To describe what is currently known about the neurobiological causes and consequences of excessive alcohol consumption among children and adolescents. Methods: A review of the relevant epidemiological and experimental studies was carried out. Results: A number of neurobiological factors affect adolescent drinking behaviour and the risk of later developing alcohol dependence. These factors are primarily genetically determined, but the risk potential develops through reciprocal interactions with the environment. alcohol-related harmful effects on the brain and its functioning can begin during adolescence. Conclusions: Avoiding drinking excessively during adolescence will facilitate brain maturational processes and reduce the risk of later developing alcohol dependence.
