  Title Treatment of Chronic Hepatitis C in Injecting Drug Users: 5 Years' Follow-Up Type Journal Article

  Abstract Aim of the Study: To assess the long-term hepatitis C (HCV) treatment outcome in former injecting drug users (IDUs). Materials and Methods: A long-term follow-up of 27 former IDUs who had been successfully treated for chronic hepatitis C was performed. These patients represented ailiDUs who had obtained a sustained virological response in a Norwegian HCV treatment trial. The patients had been treated with interferon-a alone or in combinatlon with ribavirin. At 5 years’ follow-up the 27 IDUs were retested for HCV RNA and risk behaviour for HCV transmission after treatment was assessed. In the control group ail 18 non-IDUs who had obtained a sustained virological response in the same treatment trial were included. Results: At follow-up 13-82 months (median 64) after the end of treatment only one case of probable reinfection was seen among the 27 IUDs. No reoccur- rence of HCV was observed in the control group. The IDU who was HCV RNA positive at follow-up had continued injecting drugs and reported frequent needle sharing. At follow-up HCV of genotype 1a was detected in contrast to genotype 1b before treatment indicating that this patient was reinfected with HCV. A return to injecting drug use occurred in 9 (33%) of 27 IDUs. Conclusion: The long-term outcome of HCV treatment in former IDUs was excellent. Despite frequent reinitiation of drug injection ail but 1 remained HCV RNA negative.

  Abstract Rausch, Drogenkonsum und Sucht sind Phänomene, die unmittelbar mit der Konstruktion von männlichen Identitäten verbunden sind. Drogenkonsumverhalten ist damit einzureihen in Gesundheitsrisiken allgemein, die Männer eingehen, um Männlichkeiten zu demonstrieren und permanent neu zu konstruieren. Der Zusammenhang von Gender und Drogenkonsum/-abhängigkeit ist seit 25 Jahren vorwiegend im Bereich „Frau und Sucht” erforscht und für die Praxis umgesetzt worden. Die „Entdeckung der Männlichkeit” in der Suchtforschung und -hilfe ist jüngeren Datums. Vor dem Hintergrund von zunehmender Perzeption von Gendertheorien und Gesundheitsverhalten und der stärker werdenden Notwendigkeit geschlechtsspezifisch sowohl in Prävention, Beratung, und Behandlung zu arbeiten, müssen entsprechende, Transfers in die Praxis geleistet werden. Dieser Beitrag gibt erste Hinweise für den Implementationsprozess männerspezifischer Hilfen für gefährdete bzw. abhängige Männer. Intoxication, drug use and addiction are phenomenon, which are closely associated with the construction of male identities. Drug use therefore can be understood within general health risk behaviour of men, which is meant to demonstrate or permanently (re-)construct masculinity. The interrelation of gender and drug use/addiction has mainly been studied and implemented into practice along the relationship of “women and addiction” since 25 years. The “discovery of masculinity” in drug research and drug services is of a much younger date. On the background of an increasing perception of gender theories and health behaviour and a growing necessity to work gender specifically in prevention, counselling, and treatment of drug using patients transfers into the practice have to be developed. This contribution gives first indications for this implementation process of men specific services for men at risk or dependent.

  Title Sexual uses of alcohol and drugs and the associated health risks : a cross sectional study of young people in nine European cities Type Journal Article

  Abstract Background: Young people in European countries are experiencing high levels of alcohol and drug use and escalating levels of sexually transmitted infections. Individually these represent major public health priorities. Understanding of the association between sex and substance use, and specifically the strategic roles for which young people utilise substances to facilitate sexual activity, remains limited. Methods: Respondent driven sampling methodology was used in nine European cities to survey 1,341 16-35 year olds representing youth and younger adults who routinely engage in nightlife. Participants self-completed questionnaires, designed to gather demographic, social, and behavioural data on historic and current substance use and sexual behaviour. Results: Respondents reported strategic use of specific substances for different sexual purposes. Substances differed significantly in the purposes for which each was deployed (e.g. 28.6% of alcohol users use it to facilitate sexual encounters; 26.2% of cocaine users use it to prolong sex) with user demographics also relating to levels of sexual use (e.g. higher levels of: ecstasy use by males to prolong sex; cocaine use by single individuals to enhance sensation and arousal). Associations between substance use and sex started at a young age, with alcohol, cannabis, cocaine or ecstasy use before age 16 all being associated with having had sex before the age of 16 (odds ratios, 3.47, 4.19, 5.73, 9.35 respectively). However, sexes differed and substance use under 16 years was associated with a proportionately greater increase in early sex amongst girls. Respondents’ current drug use was associated with having multiple sexual partners. Thus, for instance, regular cocaine users (c.f. never users) were over five times more likely to have had five or more sexual partners in the last 12 months or have paid for sex. Conclusion: An epidemic of recreational drug use and binge drinking exposes millions of young Europeans to routine consumption of substances which alter their sexual decisions and increase their chances of unsafe and regretted sex. For many, substance use has become an integral part of their strategic approach to sex, locking them into continued use. Tackling substances with both physiological and psychological links to sex requires approaching substance use and sexual behaviour in the same way that individuals experience them; as part of the same social process.

  Abstract Im Sport zeichnen sich Entwicklungen im Umgang mit Medikamenten und psychoaktiven Substanzen ab, die vor allem Männer betreffen. Das zeigt auch die aktuelle Diskussion über den Einsatz von Dopingmitteln im Spitzensport. Deutsche Studien im Breiten- und Freizeitsport in Fitness-Studios und in der Bodybuilding-Szene belegen darüber hinaus, dass 20-40% der sportlich Aktiven Dopingmittel wie anabole-androgene Steroide und Stimulanzien nehmen. Das Verhältnis von Männern zu Frauen, die Dopingmittel nehmen, variiert zwischen 3:1 und 9:1. Diese Daten zeigen an, dass der Konsum von Medikamenten nicht mehr allein “typisch weiblich” sondern ebenso “typisch männlich” ist. In sports, developments are observed in the use of medications and of psychoactive substances by men. It is reflected in the ongoing discussion on doping in top sports events. Empirical studies in Germany in mass sports arenas, leisure time sports activities, in fitness-clubs and in the scene of body-builders show that between 20% and 40 % of those who are active in sports use substances such as anabolic-androgenic steroids and stimulants. The relationship between men and women who use such substances varies between 3:1 and 9:1. The data show that the use of medications is not only “typical female” but as weil “typical male”.

  Abstract Biographische Statuspassagen und spezifische situative Herausforderungen in Männerwelten sind potentiell stresserzeugend sowie gesundheitsschädigend und verdeutlichen spezielle Bedürfnisse. Diese Ausgangsanalyse liefert wichtige Anhaltspunkte wenn es um männerorientierte therapeutische, präventive oder sonstige Interventionen geht. Prekäre Lebenslagen und Statuspassagen bei Männern sind überaus komplex. Während die Adoleszenzphase noch beachtliche wissenschaftliche und politische Aufmerksamkeit auf sich zieht, so werden wichtige biographische Männerproblemlagen, die auch mit Sucht und anderen risikoreichen Verhaltensweisen zusammenhängen, schlichtweg ignoriert. Aus diesem Blickwinkel werden zunächst Vatersein, Gewalt, Sexualität und Alter beispielhaft herausgegriffen. Im zweiten Teil wird zunächst der allgemeine Befund der geringeren Inanspruchnahme des Gesundheitsversorgungssystems durch Männer ganz allgemein erläutert und mit ,femininen Grundstrukturen‘ des Systems in Zusammenhang gebracht. Daran anschließend erfolgt eine Überprüfung der ,Männertauglichkeit‘ suchtspezifischer Behandlungssettings ergänzt durch Beispiele für geschlechtsrollenadäquate Behandlungsmodalitäten. Der Beitrag schließt mit einem Blick auf die Vernachlässigung des Zusammenhangs ‘Maskulinität – Risikoverhalten – Sucht‘ auch im Bereich der Prävention. Biographical status passages and specific challenges in men’s worlds tend to generate stress, health damage and point out specific needs. This assessment provides important reference points for the discussion of men-specific therapeutic, preventive or other types of interventions. Furthermore precarious life situations and status passages of men prove to be very complex. While the transition from adolescence to adulthood has met with considerable scientific and political interest, key life transitions of men, which are often linked with addiction or other high risk behaviours have been largely ignored. From this analytical perspective, firstly fatherhood, violence, sexuality and old age are highlighted, Secondly the general finding that men seek much less frequently help is discussed in view of the general health care system and its basic feminine rules and structure. Finally the fit of addiction specific treatment programs with men specific needs is taken up complemented by suggestions for gender role adequate treatment modalities. The article concludes with a look at the neglect of the relationship between ‘masculinity – risk behaviour – addiction’ in the area of prevention programs.

  Abstract Les formes de pratiques sociales des jeux sont aujourd’hui tellement diversifiées et répandues que la figure du joueur pathologique émerge désormais comme une nouvelle maladie. Il existe des arguments très forts pour inclure celle-ci dans la notion d’addiction. Cet article dresse un tableau du jeu pathologique en rappelant sa place économique et sociale et son évolution historique. Le paradoxe de cette “toxicomanie sans drogue” est qu’il s’agit évidemment d’une source de profit considérable pour l’Etat qui contrôle le système des paris et des casinos. On estime que le jeu pathologique touche 2 à 3% des adultes et qu’il s’agit d’une problématique surtout masculine. La définition du jeu pathologique (DSM-IV) met en valeur une comorbidité importante (dépression, personnalités antisociales, usage de drogues et d’alcool, trouble des conduites alimentaires). Sur le plan psychanalytique (à partir de Freud, Bergler, Fenichel) le joueur rechercherait, par une forme d’auto-punition compulsive, à résoudre l’ambivalence envers le père et l’enjeu de l’intégration de la loi, démarche comparable aux autres formes de toxicomanies. Une revue des propositions thérapeutiques est réalisée : abstention à visée thérapeutique, psychothérapies individuelles, thérapies comportementales et cognitives, thérapies de groupe, chimiothérapie psychotrope, associations et groupes d’entraide. Cet article présente une bibliographie de 32 références.
