Abstract |
Der vorliegende Beitrag gibt eine Übersicht über die bestehenden oder geforderten gesetzlichen Regelungen der medikamentösen Zwangsbehandlung in der Psychiatrie, und zwar aus supranationaler, internationaler und schweizerischer Sicht. Ausgehend von Fallbeispielen werden anschliessend vier exemplarische Situationen beschrieben, bei denen regelmässig eine psychiatrische Zwangsmedikation diskutiert wird. Es handelt sich um die Behandlung gegen einen destruktiven Willen, um die Behandlung bei fehlendem Willen, um die Behandlung gegen einen sozial störenden Willen und um die Langzeitbehandlung gegen einen chronisch destruktiven Willen. Daraus werden einige Vorschläge zur – aus der Sicht des Praktikers – sinnvollen gesetzlichen Regelung der Zwangsmedikation in der Schweiz abgeleitet. We discuss the existing or desiderated legal basis for forced medical treatment in psychiatry, in the light of supranational, international and Swiss law. Four situations in which forced medical treatment may occur are described and illustrated with case reports. A distinction is drawn between treatment against a destructive will, treatment against lack of will, treatment against an antisocial will, and long-term treatment against a chronic destructive will. Finally, proposals for improved Swiss legislation on forced psychopharmacological treatment are discussed. |

Abstract |
alcohol addiction may induce its dependence through a mechanism involving opiate receptors and opioid peptides. For these reasons, we measured ACTH, beta-lipotropin, and beta-endorphin in the plasma and cerebrospinal fluid (CSF) of 29 alcohol addicts and compared these values with those found in 8 normal volunteers. Although no significant differences existed in peripheral concentrations of the 3 peptides, alcohol addicts had beta-endorphin levels in CSF (mean +/- SE, 29.4 +/- 4.5 fmol/ml) that were 3-fold lower than those of the controls (98.4 +/- 10.5 fmol/ml; P less than 0.001) and ACTH levels 4 times higher than control values (30.0 +/- 1.8 vs. 7.4 +/- 1.1 fmol/ml in controls; P less than 0.001), while no difference was found in beta-lipotropin levels. These results suggest that alcohol addiction is associated with a marked alteration in the CSF content of proopiocortin-related peptides which may play a role in the alcohol-seeking behavior typical of the syndrome. |

Abstract |
Mit welchen geeigneten Massnahmen kann der zunehmende Verbrauch von Methylphenidat eingeschränkt werden? So die Ausgangsfrage dieses Beitrags, dessen Autor qualifizierte Fachleute für ADHS fordert, die klare Diagnosen stellen und mit individuellen Therapieplänen arbeiten können. Von den Fachgesellschaften werden neue Richtlinien verlangt. Le méthylphénidate (par ex. Ritaline) est de plus en plus employé dans beaucoup d’affections. Hormis le trouble du déficit de l’attention/hyperactivité (TDA/H), il est utilisé comme stimulant du cerveau et, plus rarement, pour de nombreux autres troubles physiques et psychiques. Pour contenir cette augmentation, des mesures appropriées sont nécessaires. Premièrement, nous avons besoin de davantage de spécialistes du TDA/H qualifiés qui posent des diagnostics clairs et travaillent avec des plans thérapeutiques personnalisés. Deuxièmement, les sociétés de discipline médicale et les associations professionnelles sont appelées à rédiger de nouvelles directives ciblées dans ce domaine, contenant notamment des dispositions en matière de formation continue, afin de garantir une prise en charge de qualité aux patients atteints de TDA/H. Enfin, il est important que chaque médecin connaisse ses limites et qu’il ne diagnostique et ne traite sous sa propre responsabilité que les affections pour lesquelles il a été formé et dans lesquelles il a acquis de l’expérience sous la supervision de spécialistes. |

Abstract |
Documented crack use emerged in São Paulo, Brazil, from 1991 onward. Therefore, it is a recent behavior among drug users. The present work draws a profile of São Paulo crack users, employing an ethnographic approach. Twenty-five crack users were interviewed on selected social and demographic characteristics, on the drug itself and its consumption, and on the consequences of this use. Crack cocaine is harmful for the user, leading within a short period to a condition of dependence. The crack users reported ultimately lapsing into “marginality” due to social isolation, neglect of bodily needs, and breakdown of family ties and other relationships. |

Abstract |
Contemporary public health emphasizes a community-based approach to health promotion and disease prevention. The evidence from the past 20 years indicates, however, that many community-based programs have had only modest impact, with the notable exception of a number of HIV prevention programs. To better understand the reasons for these outcomes, we conducted a systematic literature review of 32 community-based prevention programs. Reasons for poor performance include methodological challenges to study design and evaluation, concurrent secular trends, smaller-than-expected effect sizes, limitations of the interventions, and limitations of theories used. The effectiveness of HIV programs appears to be related in part to extensive formative research and an emphasis on changing social norms |

Abstract |
Syringe-exchange programs (SEPs) in Connecticut operate with caps on the number of syringes exchanged per visit. We investigated the effects of legislation increasing the cap on drug injectors’ access to clean syringes through the SEPs in New Haven and Hartford. The mixed design of this study included longitudinal and cross-sectional data from individuals and ecological data from program operations. Five parameters-syringe return rate, syringes per visit to the SEP, syringe reuse rate, syringe human immunodeficiency virus (HIV) prevalence, and syringe sharing-were monitored through syringe tracking and testing of SEP syringes and by interviewing injectors. Two increases in the cap-from 5 to 10 and then from 10 to 30-had little effect on the five parameters that measured injectors’ access to clean syringes. In contrast, access to clean syringes increased when the New Haven SEP first began operations, when syringes first became available at pharmacies in Hartford, and when the agency running the Hartford SEP changed. Legislation providing piecemeal increases in the cap may not, by themselves, be sufficient to increase injectors’ access to clean syringes and decrease the risk of human immunodeficiency virus transmission in this population. |
