Abstract Der nachfolgende Beitrag entstand im Rahmen der Arbeiten am Bericht “Drogenpolitische Szenarien”, den die Subkommission Drogenfragen der damaligen Eidgenössischen Betäubungsmittelkommission im Sommer 1996 erstellt hat. Eine der wesentlichen Erfahrungen, die im Laufe der Arbeiten gemacht wurden, bestand in der Dynamik des Meinungsbildungsprozesses in einer Fachkommission, in der – nicht anders als in der Bevölkerung – zum Teil stark divergierende Meinungen über den zukünftig einzuschlagenden Weg in der Drogenpolitik bestanden. Erst die vertiefte Auseinandersetzung mit den ethischen Aspekten erleichterte das Verständnis für unterschiedliche Standpunkte und führte schliesslich zu einem weitgehenden Konsens über drogenpolitische Optionen. in einer pluralistischen Gesellschaft. Die Kommission beauftragte darum ihr Mitglied Ambros Uchtenhagen, die ethischen Gesichtspunkte zu Sucht und Umgang mit Sucht darzulegen. Seine Ausfiihrungen wurden von der Kommission einstimmig genehmigt. Der Text befasst sich nicht mit konkreten drogenpolitischen Modellen, sondern zeigt die Fragen auf, die sich bei der Formulierung einer rationalen Drogenpolitik aus ethischer Sicht stellen.

Abstract This report, which provides a knowledge base for the development of methadone maintenance treatment clinical guidelines in Switzerland, reviews the world-wide literature (except for Switzerland, which is the subject of a separate investigation) on such treatment and existing guidelines. Articles on methadone maintenance treatment and concurring treatment modalities are discussed in detail, as well as existing knowledge about specific populations, such as pregnant addicts and addicts with HIV or tuberculosis. Research on prognostic factors to patients’ responses to treatment and perceptions of opiate dependent persons are also reviewed to ascertain their value to practitioners in managing methadone maintenance treatment and in assessing the best possible treatment. A final chapter describes other substitution treatments in comparison to methadone maintenance and synthesizes the information of the review into a structured information source for thinking about guideline development.

Abstract OBJECTIVES. Serial, cross-sectional trends in injecting risk behavior were studied among drug users from 1986 to 1992. METHODS. From a cohort study in Amsterdam, 616 intake visits of drug users who had injected in the 6 months preceding intake were selected. RESULTS. The proportion of drug users who reported borrowing and lending used injection equipment and reusing needles/syringes (in the previous 6 months), continuously declined from 51% to 20%, from 46% to 10% and from 63% to 39%, respectively. In multivariate analysis, it appeared unlikely that a selective recruitment of participants over time was responsible for these trends. Participants, recruited later in time, had been previously tested for human immunodeficiency virus (HIV) more often, had received daily methadone less often, and had obtained a higher proportion of new needles via exchange programs. Indications were found that (1) voluntary HIV testing and counseling leads to less borrowing, lending, and reusing equipment; and (2) obtaining needles via exchange programs leads to less reusing needles/syringes. It appeared that nonattenders of methadone and exchange programs have reduced borrowing and lending to the same extent as attenders. CONCLUSIONS. Methodologically, evaluating specific measures is difficult. However, the combination of various preventive measures in Amsterdam is likely to be responsible for the observed decrease in injecting risk behavior.

Abstract La définition du concept d’addiction apparaît de plus en plus difficile à cerner. Des toxicomanies à l’alcoolisme et au tabagisme, en passant par le jeu pathologique, les achats compulsifs, la sexualité et certains troubles du comportement… jusqu’à l’addiction, pourtant si peu virtuelle des cyberdépendants, on peut se demander quel est le point commun des addictions sans drogue et tout d’abord s’il existe. Ce questionnement est incontournable pour bâtir des stratégies de prévention et d’intervention médico-sociale adaptées. Il a également pour conséquence de penser les addictions en terme de conduite des sujets plutôt qu’une approche à partir des produits psychotropes. Ce thema de Toxibase propose d’entamer le débat à partir d’une vision conceptuelle des addictions sans drogue et d’une description nosographique et sociale de chacune d’entre elles.

Abstract Les formes de pratiques sociales des jeux sont aujourd’hui tellement diversifiées et répandues que la figure du joueur pathologique émerge désormais comme une nouvelle maladie. Il existe des arguments très forts pour inclure celle-ci dans la notion d’addiction. Cet article dresse un tableau du jeu pathologique en rappelant sa place économique et sociale et son évolution historique. Le paradoxe de cette “toxicomanie sans drogue” est qu’il s’agit évidemment d’une source de profit considérable pour l’Etat qui contrôle le système des paris et des casinos. On estime que le jeu pathologique touche 2 à 3% des adultes et qu’il s’agit d’une problématique surtout masculine. La définition du jeu pathologique (DSM-IV) met en valeur une comorbidité importante (dépression, personnalités antisociales, usage de drogues et d’alcool, trouble des conduites alimentaires). Sur le plan psychanalytique (à partir de Freud, Bergler, Fenichel) le joueur rechercherait, par une forme d’auto-punition compulsive, à résoudre l’ambivalence envers le père et l’enjeu de l’intégration de la loi, démarche comparable aux autres formes de toxicomanies. Une revue des propositions thérapeutiques est réalisée : abstention à visée thérapeutique, psychothérapies individuelles, thérapies comportementales et cognitives, thérapies de groupe, chimiothérapie psychotrope, associations et groupes d’entraide. Cet article présente une bibliographie de 32 références.
