  Abstract Das Angebot Streetwork der Ambulanten Drogenhilfe der Stadt Zürich ist in der Sekundärprävention unter anderem im Partybereich tätig. Streetwork ist bereits seit 1998 mit einem Beratungsstand und Informationsmaterial an Parties präsent. Seit Oktober 2001 bietet Streetwork im Rahmen eines Pilotprojektes Substanz- und Pillentestings als zusätzliche sekundärpräventive Massnahme im Partydrogenbereich an. In der Pilotphase von Oktober 2001 bis April 2002 wurden sechs Testings in Zürcher Trendclubs und an Zürcher Techno-Events durchgeführt. Getestet werden Partydrogen. Zu jedem Testing werden obligatorische Beratungen für jede testwillige Person vor Ort durchgeführt. Das Kantonsapothekeramt Bern, welches das Testing im Kanton Bern bereits durchgeführt hat, macht die Laboranalysen vor Ort. Die Beratungen werden von den Mitarbeiterinnen von Streetwork durchgeführt. Primäre Ziele des Testings sind die Verbesserung der oft schwierigen Erreichbarkeit der Zielgruppe, die Informationsvermittlung über die Zusammensetzung der Drogen und über die Risiken des Drogenkonsums sowie die Sensibilisierung für einen risikoärmeren Umgang mit dem Drogenkonsumverhalten. Diese Hauptziele des Testings wurden in hohem Masse erreicht. Das Testinglabor wurde jeweils zu einem Publikumsmagnet und war immer voll ausgelastet. Bedingt durch die begrenzte Laborkapazität entstanden oft längere Wartezeiten für Testwillige, die von diesen aber in der Regel in Kauf genommen und mit Beratungs- und Informationsgesprächen ausgefüllt wurden. Es zeigte sich, dass der Bedarf nach Information seitens der Zielgruppe sehr gross ist. Der Zugang zur Zielgruppe wurde durch das Testing sehr vereinfacht. Die oft auf Eigeninitiative der jugendlichen Dregenkonsumentlnnen stattfindenden Einzelberatungen vor Ort ermöglichten während dem Gespräch eine Auseinandersetzung und Reflektion mit dem eigenen, individuellen Drogenkonsumverhalten. Die Bereitschaft, offen über den eigenen Drogenkonsum zu sprechen, war sehr hoch. Die Übermittlung präventiver Botschaften. und die Information über Risiken der aktuell getesteten Substanz und anderer Substanzen hat stattgefunden. Bewusstseinsbildende Schritte konnten dementsprechend eingeleitet werden. Die Auswertungen aus den bisherigen durchgeführten Testings zeigen auf, dass vor allem jüngere Drogenkonsumentlnnen ein hohes Risikopotential mitbringen. Die Partydrogen werden bei dieser Zielgruppe häufig, regelmässig und mit wenig Kenntnissen über Gefahren und Risiken eingenommen.

  Abstract The objective of this study was to describe a 12-year (1997–2008) observation of substance-related incidents occurring at rave parties in the Netherlands, including length of visits to first-aid stations, substances used, and severity of the incidents. During rave parties, specifically trained medical and paramedical personnel staffed first aid stations. Visitors were diagnosed and treated, and their data were recorded using standardized methods. During the 12-year period with 249 rave parties involving about 3,800,000 visitors, 27,897 people visited a first aid station, of whom 10,100 reported having a substancerelated problem. The mean age of these people was 22.3+/25.4 years; 52.4% of them were male. Most (66.7%) substancerelated problems were associated with ecstasy or alcohol use or both. Among 10,100 substance-related cases, 515 required professional medical care, and 16 of these cases were life threatening. People with a substance-related problem stayed 20 min at the first aid station, which was significantly longer than the 5 min that those without a substance-related health problem stayed. These unique data from the Netherlands identify a variety of acute health problems related to the use of alcohol, amphetamines, cannabis, cocaine, ecstasy, and GHB. Although most problems were minor, people using GHB more often required professional medical care those using the other substances. We recommended adherence to harm and risk reduction policy, and the use of first aid stations with specially trained staff for both minor and serious incidents.

  Abstract Schweiz – Bern. Ergebnisse von 10 Jahren Drugchecking: Das Kantonsapothekeramt in Bern hat mehr als 1700 Substanzenproben an über 100 Events analysiert. Auf einem Poster sind nun die wichtigsten Ergebnisse der letzten 10 Jahre abgebildet. Besonderes Augenmerk ist auf die zwei Diagramme zu den Veränderungen bei XTC-Tabletten und Kokain zu richten. Die Anzahl der als XTC verkauften Tabletten, welche statt dem erwarteten MDMA andere Wirkstoffe enthielten, sank seit 2000 stetig ab, mit einem Knick in den Jahren 2008/2009. Komplementär hierzu stieg die Anzahl von Tabletten mit m-CPP. Bei Kokain ist ein kontinuierlicher Abfall des Reinheitsgrades zu verzeichnen, von durchschnittlich 60% hinunter auf durchschnittlich 30%.   Abstract The guidelines recognise that the assessment and treatment of substance misusers present forensic physicians with particular challenges that require certain skills and experience to ensure appropriate management. They stress the importance of good communication, of working closely with custody officers and of shared responsibility for the safety and care of detainees with substance misuse. In particular, they stress the importance of : the full participation of forensic physicians in all aspects and at all stages of the healthcare of detainees with substance misuse/dependence providing advice to custody officers and others involved with detainees with substance misuse/dependence comprehensive contemporaneous records appropriate sharing of information in accordance with the law and the General Medical Council’s advice on professional confidentiality being aware when making all interventions that the interests of the detainee as a patient are paramount. We believe that these guidelines will be of immense value to all practitioners in helping and supporting detainees and that they will also be useful for teaching purposes for medical and nursing staff and arrest referral officers.
