Abstract |
Eine Projektidee zur Sekundärprävention von Alkoholmissbrauch bei jungen 16- bis 25-jährigen Arbeitslosen wurde untersucht. Mit Hilfe von Einrichtungen, die sich in der Deutschschweiz primär mit Jugendarbeitslosigkeit beschäftigen, sollten junge Arbeitslose mit Alkoholproblemen gefunden und in Präventionsgruppen geleitet werden, um dort neue Kompetenzen im Umgang mit der Arbeitslosigket und dem Alkoholkonsum zu lernen. Zwei Umfagen bei entsprechenden Einrichtungen ergaben ein nur geringes Problembewusstsein und keinen Bedarf an einem entsprechenden Projekt. Arbeitslose Jugendliche in der Schweiz sind nur relativ kurz in dieser Lage. Zudem scheinen sie keine manifesten Alkoholprobleme zu haben. Diese Sichtweise stellen die Autoren der Arbeits- und Sozialbehörden in Frage. |

Abstract |
La constatation de l’existence de toxicodépendants intégrés suscite des questions importantes : comment est-il possible de consommer des drogues dures et en même temps de garder un logement et un emploi stable ? Comment gère-t-on l’appartenance simultanée à deux univers, celui de la drogue et celui de la conformité, en conflit de normes entre eux ? Des entretiens avec onze personnes, faisant partie de cette population particulière, ont permis de considérer cette forme de consommation de drogue. On a essayé d’une part de dégager les stratégies qu’il est nécessaire de mettre en place pour l’entretenir et d’autre part de comprendre les conséquences que celles-ci produisent au niveau du mode de vie des individus. L’élaboration et la réalisation de ces stratégies demandent à la personne des efforts significatifs de discipline, de planification et d’adaptation, efforts qui impliquent des conséquences importantes au niveau du mode de vie. Cette forme de consommation de drogue possède une fonction d’auto-assistance dans un but d’adaptation à la réalité, c’est pourquoi on parle de toxicodépendance d’intégration. Il s’agit donc d’un usage de drogue qui rapproche les produits psychotropes illicites des produits psychotropes licites, les deux étant caractérisés par un but identique : une meilleure insertion. Une grande question donc est suscitée par le travail d’analyse effectué : la pharmacodépendance sera-t-elle le prix social à payer dans le futur pour intégrer les marges ? Integrated drug addicts use hard drugs and, at the same time, have stable living and work conditions. Interviews with eleven people study this form of drug use : how does one manage to belong to two conflictual worlds, one concerning drug use, the other, symbolizing conformity ? And then why we refer to integrated addiction. |

Abstract |
Es gab nie eine wissenschaftliche Begründung dafür, Opiaterhaltungstherapien zu verbieten. Opiaterhaltungstherapien (nicht Substitutionsbehandlungen) waren Standard bis in die 20er Jahre, weil die meisten Opiatabhängigen Abstinenz zu einem willkürlichen Zeitpunkt nicht erreichen konnten. Nach allgemeiner medizinischer Auffassung benötigen einige Patienten dauernd Opiate. Ein Teil der Morphinabhängigen der 20er Jahre war den Heroinabhängigen des letzten Drittels des 20. Jh. sehr ähnlich. Gerade bei dieser Gruppe hatten Entwöhnungsbehandlungen schlechte Ergebnisse, und Ärzte waren wenig interessiert, diese „Psychopathen” zu behandeln. 1919 wurden in den USA und 1926 in Deutschland Opiaterhaltungstherapien verboten. Nach dem 2. Weltkrieg wurden die Ärzte vollständig aus der Behandlung Heroinabhängiger gedrängt, der ärztliche Gedanke der Schadensminderung galt nicht mehr. Behandlung war jetzt eine Mischung von Erziehung, Sozio- und Psychotherapie, bei der psychiatrische Begleitkrankheiten meist nicht angemessen behandelt wurden. Nach anfänglich euphorischer Betrachtung zeigten langfristige Katamnesen, dass die Behandlungsergebnisse so schlecht waren wie 50 Jahre zuvor. Wegen der HIV-Epidemie setzte sich das ärztliche Konzept der Schadensminderung seit Beginn der 90er Jahre langsam wieder durch. Jetzt sind Substitutionsbehandlungen (Ersatz des illegalisierten Heroins durch langwirkende Opioide in Dosierungen möglichst ohne pharmakologische Wirkung) die Standardbehandlung. Sie sind allerdings nach den Kriterien der sog. Abstinenzbehandlung und nach den staatlichen Kontrollbedürfnissen reglementiert. Das Verbot der Erhaltungstherapien hat nicht zu einer Verringerung der Zahl der Opioidabhängigen geführt, sondern zu einer Zunahme auf etwa das 50fache. Die Sterblichkeit und das HIV-und Hepatitisinfektionsrisiko sind auf das 50-100fache – verglichen mit der übrigen Bevölkerung – erhöht. History of Medical Maintenance of Opiate Dependents: There was never a scientific reason to prohibit opiate maintenance treatment. Opiate maintenance treatment (not the substitution of morphine or heroin through another opioid) was the standard treatment until the twenties. It was a general medical opinion that most of the opiate addicts could not remain abstinent at any arbitrary time and that some addicts would always need opiates. There was hardly any difference in the behaviour and appearance of a subgroup of morphine addicts from the twenties and the heroin addicts of today. Especially in this subgroup detoxification and residential treatment showed poor results. Medical doctors were not interested in treating those „psychopaths”. In 1919 the Supreme Court in the USA and in 1926 the „Reichsgericht” prohibited opiate maintenance treatment. After World War II the physicians were entirely pushed out of the treatment of heroin addicts. Treatment was now a mixture of education, socio- and psychotherapy. Harm reduction was not allowed any more. There was no appropriate treatment for psychiatric co-morbidity. At the beginning they were very euphoric about the results of this treatment, but long term follow-up showed no better results than 50 years prior. The HIV-epidemic led to a renaissance of the medical concept of harm reduction during the last 10 years. Now the standard treatment is the substitution of the illegal heroin through long-acting opioids given in a dose not showing any pharmacological effect. However this treatment is not regulated according to medical science but to the goverment´s wish to control opiates. Prohibition of maintenance treatment led to an increase in the number of addicts, the risk of death, HIV- and hepatitis- infection of addicts up to 50 times. |

Von sozialen Diensten und Einrichtungen wird erwartet, dass sie für das Gemeinwesen und für die Bürger soziale Probleme bewältigen helfen und lösen. Um diese Aufgabe zielwirksam und mit vertretbarem Aufwand wahrnehmen zu können, hat man für Humandienste die Arbeitsweise des Case Managements eingeführt. Es schafft Übersicht in der Versorgungslandschaft und Transparenz im Verfahren für alle Beteiligten und legt insbesondere für den Hilfebedürftigen Wegmarken für seine Unterstützung und sein eigenes Dazutun fest [1]. Die Perspektive Fachstellen für soziale Dienstleistungen Solothurn (Schweiz), nachstehend Perspektive genannt, ist ein Zusammenschluss verschiedener Trägerschaften in der Sozialarbeit mit Suchtmittelabhängigen. Die Fachstellen der Bereiche Beratung, Überlebenshilfe, Arbeit, Wohnen und Prävention arbeiten institutionenübergreifend nach Case Management. Organisational Development and Regional Management of Ambulatory Addict Assistance: Social services and institutions are expected to help solve social problems for the community and its citizens. In order to accomplish this task with justifiable expense in a straightforward manner, the working method of the so-called case management was introduced in the human services, improving control over social services and the perspicuity of the procedure for all the people involved by establishing milestones particularly for the persons seeking help with regard to the assistance received and their own contributions. Milestones were established for the help-seeking people with regard to their support and their own contributions. The Perspektive Fachstellen für soziale Dienstleistungen Solothurn (agencies for social services and institutions in Solothurn, Switzerland) are a pool of different organisations all working with addicts. Although the several agencies are responsible for a whole range of different social services, they collaborate by way of case management, crossing the borders of the separate institutions. |

Abstract |
Au Québec, à la fin des années 80, l’approche de réduction des méfaits s’est conjuguée à d’autres courants qui avaient déjà commencé à y exercer leur influence pour provoquer une profonde remise en question de la philosophie de traitement et des méthodes d’intervention utilisées alors dans le domaine de la réadaptation en toxicomanie. Cet article décrit plus particulièrement l’impact de l’implantation de cette approche par le Centre Dollard-Cormier en 1997. On y affirme qu’elle a constitué un point de ralliement pour tous les intervenants de ce centre et favorisé, particulièrement à travers l’expression « haute tolérance », l’adoption de valeurs telles que la flexibilité, l’accessibilité, l’acceptation de toutes les personnes qui demandent de l’aide, quels que soient leurs motifs et la précarité de leur situation. À l’aide de l’expérience des cinq dernières années, nous établissons un bilan des forces et des limites de cette approche dans un contexte de réadaptation. Au nombre des forces, on doit compter un plus grand respect des objectifs de l’usager lui-même, notamment en ce qui concerne la consommation de substances psychoactives, une transformation des services pour les rendre plus flexibles et accessibles, et un préjugé favorable aux plus démunis. Au nombre des difficultés, on mentionnera l’adaptation de nos méthodes d’intervention à des objectifs variés en ce qui concerne la consommation et la conciliation de la haute tolérance avec un cadre thérapeutique qui impose des limites. Si l’approche de réduction des méfaits nous a appris à accepter toute amélioration de la situation des personnes toxicomanes comme un résultat légitime et valable en soi, notre mission de réadaptation nous invite à ne pas perdre de vue l’objectif qu’un grand nombre parmi elles poursuit néanmoins : arriver à se libérer de leur dépendance à ces substances, par l’abstinence ou autrement, et retrouver la maîtrise de leur vie. In the late 80’s, in Quebec, the harm reduction approach was mixed with other currents that had already started to influence and renew the philosophy of treatment and the methods of intervention used at the time in the field of drug abuse rehabilitation. This article will discuss the impact of the 1997 implementation of this approach at the Centre Dollard-Cormier. We assert that the care-givers of the centre rallied to the approach and that it facilitated, especially through the expression of “high tolerance”, the adoption of such values as flexibility, accessibility and acceptance of all persons who request help, whatever their motives or the precariousness of their situation. With a five-year background, we can assess the strengths and limits of the approach in the context of rehabilitation. Concerning the strengths, we must observe a higher respect of the user’s objectives, in particular those that concern the use of psychoactive substances, a transformation towards more flexible and accessible services, and a favourable prejudice towards the less fortunate. As for the difficulties, we will mention the adjustments made to our methods of intervention to meet the varied objectives of substance use and to conciliate high tolerance with the therapeutic framework. Even though the harm reduction approach has taught us to appreciate any improvement in the situation of addicted persons as a legitimate result, valid by itself, our mission requires us not to lose sight of the large number users who nevertheless pursue the objective of liberating themselves from their dependence, either by abstinence or otherwise, and take back control of their lives. En el Quebec, al final de los años 80, el enfoque de reducción de perjuicios se conjugó con otras corrientes que ya habían comenzado a ejercer su influencia, para provocar una puesta en duda profunda de la filosofía de tratamiento y de métodos de intervención que estaban en uso en el sector de la readaptación de la toxicomanía. Éste artículo describe más particularmente el impacto que tuvo la adopción de éste enfoque en el Centro Dollard-Cormier en 1997. Se afirma que se ganó la adesión de todos los trabajadores sociales que intervenían en ése Centro, y que particularmente la expresión « alta tolerancia » favoreció la adopción de valores tales como la flexibilidad, la accesibilidad, la acceptación de todas las personas que piden ayuda, cuales sean sus motivos y la precaridad de su situación. Gracias a la experiencia de los cinco últimos años, un balance de los puntos fuertes y de los límites de éste enfoque fue establecido en el contexto de la readaptación. Entre los puntos fuertes, se debe contar un respeto mayor de los objetivos proseguidos por el mismo usuario particularmente en lo que atañe al consumo de substancias psicoactivas, una transformación de los servicios para que se vuelvan más flexibles y accesibles y un prejuicio favorable a los más despojados. Entre las dificultades encontradas, se debe mencionar la adaptación de nuestros métodos de intervención a variados objetivos relativos al consumo y la conciliación de la alta tolerancia con un marco terapéutico que impone sus límites. Si el enfoque de reducción de perjuicios nos enseñó a aceptar toda mejora de la situación de las personas toxicómanas como un reslutado legítimo y válido de por sí, nuestra misión de readaptación nos invita a no perder de vista el objetivo proseguidoa pesar de todo por gran número de entre ellas : llegar a liberarse de su dependencia a estas substancias, por abstinencia o de otro modo, y volver a tener el control de su vida. |

Nous avons mené des entrevues en profondeur auprès de 24 jeunes (15 à 22 ans) recrutés avec l’aide d’organismes communautaires et par la technique de « snowballing ». Au moment de la première injection, ces jeunes sont peu préoccupés par les risques d’infection et s’inquiètent davantage du risque de développer une dépendance. Cependant, ceux qui persistent sont conscients des risques associés au « partage » de seringues et considèrent qu’il s’agit d’une pratique à éviter dans une ville où les programmes de prévention fournissent l’accès gratuit aux seringues. Ce n’est toutefois pas le cas pour les autres matériels d’injection. Les contextes de vulnérabilité par rapport au « partage » se constituent au carrefour de plusieurs facteurs qui sont à la fois d’ordre individuel et social. Certains moments critiques dans la trajectoire de vie des jeunes par rapport à la consommation de drogues et à la vie dans la rue ainsi qu’un degré élevé d’intimité entre partenaires d’injection les fragilisent pour ce qui est des pratiques non sécuritaires. The “sharing” of Injection Equipment Among Young Injection Drug Users in Montreal The goal of this study was to identify the contexts in which street youth who inject drugs share their injection equipment. In-depth interviews were conducted among youth recruited through community groups and utilising a “snowballing” technique. We interviewed 24 youth (aged 15 to 22 years). At the time of their first injection, the youth are not very concerned with the risks of infection but worry more about developing an addiction. However, youth who continue to inject are aware of the risks associated with needle sharing and consider that, in a city where prevention programmes provide free access to syringes, this practice should be avoided. This is not the case for other injection equipment. Contexts of vulnerability associated with sharing originate when personal and social factors intersect. Certain critical moments in a youth’s life related to drug consumption and living in the streets, as well as the high level of intimacy among injection partners make youth more vulnerable to unsafe practices. El «compartimiento» de los materiales de inyección entre los jóvenes adictos de drogas inyectables en Montreal Este estudio pretendía identificar los contextos en los cuales el compartimiento del material de inyección se efectúa entre los jóvenes de la calle que se inyectan drogas. Entrevistas en profundidad han sido realizadas acerca de jóvenes reclutados a partir de organismos comunitarios y por la técnica de « snowballing ». Hemos interrogado 24 jóvenes (de 15 a 22 años). En el momento de la primera inyección se preocupan poco de los riesgos de infección, pero se inquietan sobre todo del riesgo de desarrollar una dependencia. Sin embargo, los jóvenes que persisten en la inyección están concientes de los riesgosd asociados al hecho de compartir las jeringas, y consideran que se trata de una práctica que se debe evitar en una ciudad dónde los programas de prevención procuran acceso gratis a las jeringas. No es el caso para los otros materiales de inyección. Los contextos de vulnerabilidad por respeto al hecho de compartir éste material, se constituyen en la encrucijada de varios factores a la vez de orden individual y social. Ciertos momentos críticos en la trayectoria de vida de los jóvenes en miras de su consumo de drogas y de su vida en la calle, así como un grado elevado de intimidad entre « compañeros de inyección » fragilizan a los jóvenes por respecto a prácticas poco seguras. |
