Abstract |
Dans la perspective de tenter de diminuer le nombre d’overdoses mortelles à Genève, une formation aux mesures de réanimation cardio-respiratoire a été expérimentée. Introduite il y a trois ans dans des lieux stationnaires de court et moyen terme, elle est animée par une équipe pluridisciplinaire. Elle a deux objectifs : l’aide à la survie (la réduction des dommages) et la réduction des risques. A ce jour, 94 personnes ont suivi l’ensemble de la formation. Des outils d’évaluation permettant de mesurer l’efficacité de la formation et les performances des participants à court et à moyen terme ont été mis en place. Une démarche de recherche-action est en cours. Les résultats observés sont très satisfaisants. Associée à d’autres mesures de réduction des risques, la formation a permis de diminuer le nombre d’overdoses mortelles. L’intérêt réside dans le fait de considérer l’usager de drogue comme acteur de la prévention. |

Abstract |
Fragestellung: Bisher ist wenig darüber bekannt, welche internen bzw. externen Effekte eine erfolgreiche Zertifizierung hat. Hier setzt eine Studie an, die Anfang 2007 von der Deutschen Gesellschaft für Qualitätsmanagement in der Suchttherapie e. v., Kassel (de-Qus) in Zusammenarbeit mit Prof. Dr. Aloys Prinz (Gesundheitsökonom am Fachbereich Wirtschaftswissenschaften an der WWU Münster) durchgeführt wurde. Ziel der Studie war es, die internen und externen Auswirkungen einer Zertifizierung zu untersuchen. Methodik: Zertifizierte deQus-Mitglieder wurden anhand eines Fragebogens nach ihren anfänglichen Erwartungen bzgl. interner und externer Effekte einer Zertifizierung und nach den tatsächlich erzielten Wirkungen gefragt. Ergebnisse: Die Ziele der Einrichtungen, mit der Zertifizierung einen insbesondere von den Leistungsträgern anerkannten Qualitätsnachweis zu erbringen, wurden nicht erreicht. Die intern realisierte Qualitätsentwicklung überstieg jedoch die ursprünglichen Erwartungen. Schlussfolgerungen: Die gesetzliche Zertifizierungspflicht und die diesbezüglichen Vorgaben der Bundesarbeitsgemeinschaft für Rehabilitation (BAR) lassen erwarten, dass zukünftig eine Zertifizierung auch als Nachweis eines guten Qualitätsniveaus gelten wird. Aim: Because little was known about the internal and external effects of successful certification, this study aimed to determine what these effects are. Method: Certified members of the German Society of Quality Management in Addiction Therapy («deQus») were asked to complete a questionnaire on their initial expectations about the internal and external effects of obtaining certification and the success that they actually achieved. Results: The goal of the institutions to provide proof of quality to certification agencies was not achieved. However, the improvement in quality that they realized exceeded initial expectations. Conclusions: The legal obligation set by the German Federal Study Group for Rehabilitation (BAR) that institutions obtain certification has led to the expectation that in future certification will be accepted as proof of a high level of quality. |

Abstract |
alcohol addiction may induce its dependence through a mechanism involving opiate receptors and opioid peptides. For these reasons, we measured ACTH, beta-lipotropin, and beta-endorphin in the plasma and cerebrospinal fluid (CSF) of 29 alcohol addicts and compared these values with those found in 8 normal volunteers. Although no significant differences existed in peripheral concentrations of the 3 peptides, alcohol addicts had beta-endorphin levels in CSF (mean +/- SE, 29.4 +/- 4.5 fmol/ml) that were 3-fold lower than those of the controls (98.4 +/- 10.5 fmol/ml; P less than 0.001) and ACTH levels 4 times higher than control values (30.0 +/- 1.8 vs. 7.4 +/- 1.1 fmol/ml in controls; P less than 0.001), while no difference was found in beta-lipotropin levels. These results suggest that alcohol addiction is associated with a marked alteration in the CSF content of proopiocortin-related peptides which may play a role in the alcohol-seeking behavior typical of the syndrome. |

Fragestellung: Gibt es ein Cannabisstereotyp, das sich auf die leistungsbezogenen Erwartungen von Lehrern auswirkt? Methodik: 285 Probanden beurteilten in einem länderübergreifenden Experiment einen 16-Jährigen anhand verschiedener Aussagen zum schulspezifischen Verhalten. Der Junge wurde zusätzlich entweder als gelegentlicher Cannabiskonsument oder als “Schwarzfahrer” beschrieben. Ergebnisse: In der Bedingung des gelegentlichen Cannabiskonsumenten wurde der Schüler als weniger leistungsbereit, weniger leistungsfähig und weniger kooperations bereit eingestuft. Schlussfolgerungen: Gleichwohl empirische Belege eines amotivationalen Syndroms fehlen, hält sich das Bild des antriebslosen “Kiffers” und beeinflusst die Erwartungen von Lehrern. Aims: Is there a cannabis-user stereotype that influences teachers expectations about pupils performance in school? Method: In a cross-national study, 285 respondents evaluated a 16-year-old boy on a list of attributes depicting a cannabis-user stereotype and on several statements about the boys school-related behaviour. He was described either as an occasional cannabis user or as a fare dodger. Results: When described as a cannabis user, the pupil was rated as less ready to perform, less able to perform, and less ready to cooperate. Conclusions: Despite the lack of evidence for an amotivational syndrome, the stereotype of the listless pothead seems to persist and to influence teachers expectations about their pupils. |

Fragestellung: Welche neurobiologischen Erkenntnisse zu Ursachen und Folgen übermäßigen Alkoholkonsums bei Kindern und Jugendlichen sind gegenwärtig bekannt? Methodik: Überblick über einschlägige epidemiologische und experimentelle Studien. Ergebnisse: Eine Reihe neurobiologischer Faktoren beeinflusst das Trinkverhalten Jugendlicher und das Risiko für spätere Alkoholabhängigkeit. Sie sind größtenteils genetisch beeinflusst, entfalten ihr Risikopotential jedoch erst durch wechselseitige Interaktion mit Umwelteinflüssen. Bereits im Jugendalter können Alkoholfolgeschäden am Gehirn und seiner Funktion auftreten. Schlussfolgerungen: Das Vermeiden übermäßigen Alkoholkonsums im Jugendalter begünstigt den noch im Ablauf befindlichen Prozess der Gehirnreifung und verringert die Gefahr späterer Alkoholsucht. Aim: To describe what is currently known about the neurobiological causes and consequences of excessive alcohol consumption among children and adolescents. Methods: A review of the relevant epidemiological and experimental studies was carried out. Results: A number of neurobiological factors affect adolescent drinking behaviour and the risk of later developing alcohol dependence. These factors are primarily genetically determined, but the risk potential develops through reciprocal interactions with the environment. alcohol-related harmful effects on the brain and its functioning can begin during adolescence. Conclusions: Avoiding drinking excessively during adolescence will facilitate brain maturational processes and reduce the risk of later developing alcohol dependence. |

Abstract |
Auf Basis internationaler Standards orientiert sich die Gesundheitsversorgung Gefangener auch in Deutschland am „Äquivalenzprinzip“ (Gleichwertigkeit der medizinischen Versorgung innerhalb der Gefängnisse mit der im jeweiligen Land vorgehaltenen medizinischen Versorgung). Vor diesem Hintergrund werden anhand ausgewählter Gesundheitsprobleme nicht nur vereinzelte Angleichungs- und Umsetzungsprobleme in der Gesundheitsversorgung Gefangener verdeutlicht, sondern auch strukturelle Probleme benannt, die aus dem von der GKV entkoppelten Parallelsystem der gefängniseigenen „Gesundheitsfürsorge“ resultieren. Die vorliegende Übersichtsarbeit dient als Grundlage für Vorschläge zum Umdenken und zur Reorganisation der intramuralen Gesundheitsdienste. On the basis of international standards, health care in German prisons has been oriented along the principle of equivalence (equity of health care delivery for inmates compared with the health care delivery in the community). Against this background, selected health problems demonstrate not only isolated problems in adaptation and implementation of health care delivery, but also structural problems resulting from the parallel system of prison health care, which is separated from the general German health care insurance system. This review serves as a basis to present suggestions to rethink and reorganize prison-based health care services. |

Abstract |
This report consolidates previous recommendations and adds new ones for preventing and controlling infections with hepatitis viruses in correctional settings. These recommendations provide guidelines for juvenile and adult correctional systems regarding 1) identification and investigation of acute viral hepatitis; 2) preexposure and postexposure immunization for hepatitis A and hepatitis B; 3) prevention of hepatitis C virus infection and its consequences; 4) health education; and 5) release planning. Implementation of these recommendations can reduce transmission of infections with hepatitis viruses among adults at risk in both correctional facilities and the outside community. These recommendations were developed after consultation with other federal agencies and specialists in the fields of corrections, correctional health care, and public health at a meeting in Atlanta, March 5–7, 2001. This report can serve as a resource for those involved in planning and implementing health-care programs for incarcerated persons. |

Abstract |
Fragestellung: Die Einschätzung der Bedeutung der AWMF-Leitlinien für die Zukunft der Suchthilfe aus Sicht der Wissenschaft, Suchthilfeverbände, Leistungsträger und Gesundheitspolitik wurde erfragt. Methodik: Vertreter aus Gesundheitspolitik, Suchtforschung, ambulanter und stationärer Suchthilfe und Leistungsträgern wurden aufgefordert, anhand eines vorgegebenen Fragenkatalogs ihre Einschätzung zu den Leitlinien zu formulieren. Ergebnisse: Die Erarbeitung der Leitlinien werden als Fortschritt bewertet, die Möglichkeiten der praktischen Umsetzung kontrovers und die Bedeutung EBM-Kriterien genügender Studienergebnisse für den Versorgungsalltag als unzureichend beurteilt. Schlussfolgerungen: Ergebnisse der Umfrage zeigen die Notwendigkeit, die Realität der Suchtkrankenversorgung und Bedeutung des Versorgungsauftrags wissenschaftlich stärker zu berücksichtigen und für die praktische Umsetzung ausreichend Ressourcen zur Verfügung zu stellen. |
