Abstract Kinder sind unsere Zukunft und sie haben ein Recht auf Bildung. Die Forschung hat nachdrücklich aufgezeigt, das die ersten Lebensjahre am lernbedeutsamsten sind und den Grundstein für den späteren Bildungs- und Lebenserfolg legen. Was in dieser Phase versäumt wird, kann später kaum aufgeholt werden. Deshalb liegt der Schlüssel zur Chancengerechtigkeit in der frühkindlichen Bildung. International und auch in der Schweiz hat die frühkindliche Bildung, Betreuung und Erziehung in letzter Zeit grosse Aufmerksamkeit erlangt. Vom Eltern-, über das Schul- bis zum Bundeshaus gibt es eine grosse Übereinstimmung, dass wir diesem Bereich eine weit grössere Aufmerksamkeit schenken müssen als bisher. Im Frühjahr 2009 fand dazu eine internationale Tagung an der Universität Fribourg statt, unter dem Matronat von Prof. Margrit Stamm und in Zusammenarbeit mit der Schweizerischen UNESCO-Kommission. Das Leitthema dieser Tagung konzentriert sich auf die Frage, wie kleine Kinder erzogen, betreut und gebildet werden sollen, damit sie optimal für einen erfolgreichen Lebensweg vorbereitet sind und damit alle Kinder die gleichen Bildungs- und Integrationschancen bekommen Dieser Band wird die Ergebnisse der Tagung zusammenfassen; Kontroversen aufzeigen, die sich rund um die Frage ergeben, inwiefern Bildung im Vorschulalter stattfinden soll und schliesslich offene Fragen identifizieren, die Grundlage für eine vertiefte Auseinandersetzung bilden.

Abstract Phase I of the Zurich Prometheus Study is a cross-sectional study focusing on an up-to-date serology for HIV and hepatitis B/C and associated risk factors for all clients in four participating clinics offering opiate substitution in Zurich, Switzerland. The mean age of the 603 respondents is 30.7 years (SD=6.2), and 38% of them are women. Seventy-five percent of the respondents have a history of injecting drug use (IDU), and over half have injected within the past six months. Lab-confirmed seroprevalence for HBV (50%) and HCV (57%) is twice that of HIV (24%). There is an 80% risk reduction for all three viral infections among those starting IDU after 1991—when harm reduction efforts were in full swing—compared to those who began before 1988—before clean needles were widely available. These findings suggest a strongly protective effective of harm reduction measures. But while a stabilization in HIV prevalence at 15% can be seen among drug users who started injecting after 1991, prevalence rates for HBV and HCV still remain several times higher. The prevalence data in this study support data showing continued high incidence rates for HBV and HCV, even among new injectors in the harm reduction era.

Abstract Results of needle and syringe exchange programmes (SEPs) in prisons based on 10 years experience in Switzerland, Germany, Spain and Moldova are presented. SEPs have been introduced in 46 European prisons, predominantly as pilot projects. Forty-three of these projects were still operating at the time of writing. In 11 prisons, SEPs were evaluated to assess feasibility and efficacy. Results did not support fears that commonly arise in the start-up of implementation of SEPs. Syringe distribution was not followed by an increase in drug use or injection drug use. Syringes were not misused, and disposal of used syringes was uncomplicated. Sharing of syringes among drug users reduced. Based on these experiences, it can be concluded that in these settings harm reduction measures, including syringe exchange, were not only feasible but efficient. Despite these positive results, syringe exchange in prison is far from general acceptance. However, a governmental decree released in Spain in 2001 that all prisons in the country are required to provide drug users with sterile injection equipment may lead to a breakthrough of this harm reduction measure in the future. The discrepancy concerning the success of SEPs in prisons on the one hand and its low acceptance on the other hand is striking. Suggestions for the installation of SEPs in prison are given to assist a more objective discussion.

Abstract Die HIV-Nachweise gingen in den Jahren 1995–1999 in der heterosexuellen Transmissionsgruppe weniger stark zurück als in der homosexuellen Gruppe und bei Drogen injizierenden Personen. Der Hauptgrund liegt in einer Zunahme bei Personen aus dem aussereuropäischen Ausland, vor allem mit Nationalität in Ländern südlich der Sahara. Bei einem Teil dieser Fälle könnte die Infektion im Ausland stattgefunden haben. Zugangsbarrieren zu Informationen und Versorgungssystemen in der Schweiz könnten das HIV-Ansteckungsrisiko für diese Personengruppe ebenfalls erhöhen. Mit weitergehenden Untersuchungen muss geklärt werden, welche der verschiedenen Möglichkeiten vorwiegen, damit gezieltere Präventionsmassnahmen in Zusammenarbeit mit den betroffenen Personengruppen eingeleitet werden können.
